Urogenital tuberculosis is still a problem of health not only in our country but in the whole world, especially with the pandemia of AIDS. The gravity of the attack lies in the risk of renal insufficiency and infertility. From 07-12-03 to the 13-03-2008, 32 suffering patients of urogenital tuberculosis were treated in our service of urology UHC Mohamed VI. The patient was left again into: 19 men (59, 3%) and 13 women (40, 6%) whose average age is 45, 5 years, with extreme energy of 25 to 70 years. The UIV showed a renal muteness in 25 % of the cases, an urétérohydronéphrose in 40% of the cases, a small bladder in 20 % of the cases, the renal function summer disturbed among 5 patients. The search for BK in the urines be positive in 3, 1 % of the cases. Histology was positive in 13 of the cases. All the patients were treated by antibacillaires according to the 9 months, with diagram 2SHRZ /7RH. Surgery be necessary in 93, 7%: nephrectomy 23, 3%, epididymectomy 30%, surgery of reconstitution in 20 % of the cases. The prevention of this affection rests on the generalization of vaccination BCG, the adequate treatment of the pulmonary tuberculosis and the early diagnosis of the urogenital attack.

Antimicrobial resistance (AMR) represents one of the major global health issues today. Infections with extensively drug-resistant (XDR) and pan drug-resistant(PDR) bacteria are associated with prolonged treatment periods and high lethality, owing to the limited and sometimes absent therapeutic options. Due to the misuse of antibiotics by means of over-prescription and their abuse in agriculture, AMR is continuously spreading and causing dangerous outbreaks that prompt establishing persistent and accurate surveillance. Hospitals and other healthcare establishments are fertile ground fordrug-resistant bacteria to flourish, especially biofilm-forming bacteria such as the WHO top priority pathogen Acinetobacter baumannii. In this study, we utilize next-generationsequencing (NGS) as a fast and effective diagnostic tool to gain insight into the genomic diversity and clonality of an A. baumannii outbreak at a tertiary care center in Beirut, Lebanon. Methods: A total of 54 A. baumannii isolates from ICU patients and from the hospital environment collected from AUBMC Clinical Microbiology Laboratory were screened for their susceptibility using the Broth Micro-dilution assay (BMD) against 12 different antimicrobials from different antimicrobial classes. For the purpose of determining AMR genes, sequence type (ST) and international clones (IC), whole-genome sequencing(WGS) using Illumina sequencing was performed on all these isolates. Results: According to CLSI guidelines, two A. baumannii isolates were found to be resistant to colistin. All isolates showed resistance to Bactrim. Resistance rates were 98%for imipenem, meropenem, tazocin, ciprofloxacin, levofloxacin, ceftazidime, and cefepime, while resistance to gentamicin and amikacin accounted for 93% and 94%,respectively. Furthermore, 96% of tested A. baumannii isolates can be categorized asXDR, 2% as PDR while 2% were susceptible to antibiotics. Additionally, many antimicrobial resistance genes were detected, including the carbapenem resistance geneOXA-23. All our isolates were of ST 2, with 93% of them belonging to IC 2.Conclusion: The study focused on A. baumannii isolates and found that those belonging to ST-2 and IC-2 were the most prevalent. These isolates were clustered together and had similar antimicrobial resistance genes. WGS was crucial in identifying these differences and highlighting the importance of advanced techniques in tracking and identifying outbreaks accurately. The finding that most isolates detected belongs to IC-2 and ST-2carrying blaOXA-23 suggests that the current outbreak strain might have originated from older isolates dating back to 2014-2020. This study emphasizes the importance of 3utilizing NGS in monitoring the antibiotic-resistant pathogens as a way to prevent and control outbreaks.

Objective and Background :Pharmaceutical care is an important aspect of modern pharmacy practice, yet little is known about how pharmacists perceive and implement it in their daily work. This cross-sectional study to examine the perspectives of community pharmacists in Palestine regarding the concept of pharmaceutical care, the frequency of its application, and the obstacles to its application. Methodology: A self-administered survey was conducted among pharmacists working in community pharmacies. The survey assessed describe the community pharmacists’ expectation to provide pharmaceutical care practice. A total of 310 pharmacists completed the survey. Pharmaceutical Care Intention scale was used to assess care expectation among community pharmacists. Results:The result of our study indicated that the most Drug Related Problems reported by the participant pharmacists were adverse drug reactions (46.0%), followed by dose related problem (44.0%), need for additional drugs for untreated conditions (33.0%), and patients did not receive the drug (26.0%). One third of the pharmacist expect to “Ask patient information about their medications, diseases and medical history” in 40-60 % of the patients. More than half of the respondents (53%) “Discuss the appropriate use, side effects of drugs to prevent potential drug-related problems” in 60-100% of their patients. Comparatively, the pharmacists directly contact the primary physician to solve drug related problems and monitor patients’ outcomes in only 0-20% and 20-40% of the patients. In terms of the variable perceived moral obligation , the pharmacist felt very responsible to identify any potential drug related problems and prevent it (mean= 4.0±1.14) and intervene to solve any exist drug related problem (mean= 3.9 ±1.28). To a less extent pharmacist felt responsible to collect patient-specific information (mean= 3.1 ±1.1). iv For subjective norm, pharmacists were somewhat more likely to agree that physicians and other healthcare would approve of them providing pharmaceutical care services. However, 30 % still agreed that patients would approve of them providing pharmaceutical care services. Fewer respondents agreed that other pharmacists they knew how to provide pharmaceutical care services to their patients. In terms of attitude, the respondents tended to agree that providing pharmaceutical care services was very likely for the pharmacist to attract more patients to their pharmacy (4.15 ±1.06), it would be a significant benefit to patients’ health outcomes (4.40 ±0.84) and it was a valuable opportunity for profession to contribute to the community, even if this increased their workload (3.97 ±1.12). They were less likely to agree that patient’s appreciation of the pharmacist’s value (3.87± 1.43).The multivariate analysis found only the constructs of attitude (β = 0.399, P= 0.001), and perceived behavioral control (β = 0.410,P = 0.001) to be significant predictors of pharmacists’ intention to provide pharmaceutical care. Education level was the only demographic variable that was found to be significant predictors of pharmacists’ intention to provide pharmaceutical care (β = 0.812, P = 0.032). None of the other demographic or practice setting variables was significant at P < 0.05. The adjusted R2 value was equal to 0.684, which means that 68.4% of the variation in intent was explained by the independent variables. Conclusion:The respondents showed a moderate intention to practice pharmaceutical care. However, some barriers to the implementation of pharmaceutical care were identified, including lack of time, workload, and lack of space. The findings suggest that there is a need for more education and training for pharmacists to enhance their knowledge and skills in providing pharmaceutical care.
