This study was conducted to determine the activity of Portulaca oleracea gainst Pseudomonas aeruginosa isolated from postoperative wound infections in Gedarif Teaching Hospital during the period between June 2013 to June 2016, three hundred and fifteen (bacterial swab) collected, ninety six isolates of Pseudomonas aeruginosa were isolated ( 96 30%.). Antimicrobial susceptibility test of commonly used antibiotics (Piperacillin, Ceftazidime, Gentamicin, Ciprofloxacin and Doxycycline against P. aeruginosa was done. The results were found to be sensitive to to Ciprofloxacin (81.84.4%) and followed by Ceftazidime (78. 81.3%), piperacillin (69. 71.9%), Gentamicin (66. 68.8%) and Doxycycline (12. 12.5%). Sensitivity test was carried out using the extracts of the leaves and stems of Portulaca oleracea against Pseudomonas aeruginosa. The result of the sensitivity test showed that water extracts was less effective than ethanolic extracts. The minimum inhibitory concentration of ethanolic extracts and water extracts from the leaves and stems was found to be 62.5 mg/ml and 125 mg/ml respectively. The in vivo effect of ethanol and water extracts from leaves and stems of Portulaca oleracea were determined by used cream form on infected wound of white rats. A control positive group of infected rats were treated with known antibiotic; a control negative group of the rats, which were left without treatment. The outcome showed that untreated group (negative control) remained without wound healing while the treated groups by extracts of Portulaca oleracea and antibiotics ( control positive) were cured within twelve to fifteen days. The ethanolic extracts of Portulaca oleracea were separated by preparative thin layer chromatography systems into four compounds, one of these four compounds was found effective against Pseudomonas aeruginosa with inhibition zone of 12.5 ± 0.5. The Minimum inhibition concentration (MIC) was determined for bioactive compound, were found to be 10 µg. the molecular weight of bioactive compound was determined using Liquid Chromatography-Mass Spectrometry system was found to be m/z 269.19 which is correspond to molecular weight of apigenin with chemical formula C15H10O5. This further confirms that the bioactive compound is apigenin which is one of the flavonoids of Portulaca oleracea
