Le genu varum est une déformation primitive ou secondaire, caractérisée par une angulation latérale interne de la jambe par rapport au fémur. Il en résulte une dégradation cartilagineuse puis osseuse du compartiment fémoro-tibial interne qui augmente à son tour l’importance du varus. L’ostéotomie tibiale curviplane de valgisation est une technique précise qui permet de corriger le défaut de l’axe dans le plan frontal et de diminuer les contraintes biomécaniques excessives sur le compartiment fémoro-tibial interne. Notre étude est rétrospective réalisée sur une période de 5 ans (du janvier 2005 au décembre 2009), portant sur 14 cas de genu varum traités par ostéotomie curviplane. L’âge moyen de nos patients était de 51 ans avec des extrêmes allant de 22 ans à 75 ans, avec prédominance féminine. La symptomatologie était marquée essentiellement par la gonalgie et la diminution du périmètre de la marche. L’imagerie avait pour but la mesure des déviations angulaires ainsi que la correction chirurgicale et la surveillance de l’évolution. Tous nos patients avaient présentés une gonarthrose médiale dont 78% étaient stade 2 selon la classification d’Ahlback, et la déviation en genou varum était en moyen de 10° avec des extrêmes allant de 6 à 20°. L’ostéotomie curviplane à concavité inférieure a été réalisée chez tous nos patients et la fixation était par des agrafes dans 92% des cas. Au terme de notre étude, les résultats anatomiques et fonctionnels étaient très encourageants : très bons résultats dans 72%, bons résultats dans 14% et résultats moyens dans 14% des cas.

- The genu varum is a primary or a secondary deformation, characterized by an internal lateral angulation of the leg from the femur. A cartilage, then a bone deterioration of the internal femoro-tibial compartment, which increases the importance of varus, comes as a result of this deformation. The valgus tibial osteotomy curviplanar is a precise technique that corrects the defect of the axis in the frontal plane and reduce the excessive biomechanical constraints on the internal compartment femorotibial. Our retrospective study was conducted over a period of five years (from January 2005 to December 2009) on 14 cases of genu varum treated by osteotomy curviplanar. The average age of our patients was 51 years ranging from 22 to 75 years with a female predominance. The symptomatology was mainly marked by the knee pain and lower perimeter of the march. The radiological assessment aims to measure angular deviations and surgical correction and evolution monitoring. All of our patients had a medial gonarthrosis which 78% of them were at stage 2 according to the classification of Ahlbäck, and knee varus deviation averaged 10 ranging from 6 to 20°. Osteotomy curviplanar with inferior concavity was applied on all patients and the fixation was with staples in 92% of cases. At the end of our study, the anatomical and functional results were very encouraging: very good results in 72%, good results in 14% and average results in 14% of case
