Background: The major factor associated with the endodontic failure is the persistent microbial infection in the root canal system and /or in the peri-radicular area. It is important to remove as much sealer and gutta-percha as possible during retreatment to uncover remnants of necrotic tissue or bacteria that might set as the antigenic source. Aims of Study:Evaluation of efficacy and safety of hand and two rotary retreatment systems (R Endo™, ProTaper®UniversalRetreatment) in removing root canal filling materials. Evaluation of efficacy and safety of using solvent, heat and ultrasonic appliance with hand retreatment system (Gates-Gliddendrills + H Files + K Flexile Files). Evaluation of efficacy and safety of using solvent, heat and ultrasonic appliance with two rotary retreatment systems (R Endo™, ProTaper®Universal Retreatment).Materials & Methods: This research consisted of an in-vitro and a clinical study.In-vitro study:A total of 144extracted human single-canal teeth were de-coronated leaving root segments with an average length of (21 ± 0.5) mm. Teeth were then instrumented using 0.06 taper K3™rotary instruments up to ISO size #35, the canals were obturated with gutta-percha and SEALITE™Regular sealer using lateral condensation technique. Teeth were then stored at 37°C in 100% relative humidity for four weeks to allow complete setting of the sealer. Teeth were then divided into three main groups of 48 teeth each, with Group 1:samples were subjected to R Endo™rotary files, Group 2:samples were subjected to ProTaper®Universal Retreatment Rotary Files and Group 3:samples were subjected to (Gates-Gliddendrills + H Files + K Flexile Files). The main groups were further divided into four subgroups (A, B, C and D) of 12 teeth each according to the adjunct appliance associated to files used for the removal of filling materials: Subgroup A:nothing, Subgroup B:heat, Subgroup C:solvent and Subgroup D:ultrasonic activation. During retreatment الملخَّص viiprocedures, teeth were attached to model (which is specially designed for this study) allowed to fix tooth and to collect the apically extruded filling materials, which was weighed later to 10-4precision using a microbalance. Time required to reach full root canal working length was recorded. Then, teeth were longitudinally split into halves and photographed. Cleanliness of root canal wall was evaluated according to Hulsmann's scoring system. Complications were observed.Clinical study:This study was performed on 135single-canal anterior and premolar teeth of 96 patients (28 male & 68 female)with ages ranging between 20 and 56 years old, with an average age 38.7 years. The study was confined to the patients with asymptomatic teeth which are indicated to rootcanal retreatment. Teeth were randomly assigned into three main groups of 45 teeth each according to the files used for removing filling materials: Group 1:R™Endo rotary files, Group 2:ProTaper®Universal Retreatment Rotary Files and Group 3:(Gates-Gliddendrills + H Files + K Flexile Files). Each main group was divided into three subgroup of 15 teeth each, according to the adjunct appliance combined with files in removing filling materials: Subgroup A:nothing, Subgroup B:heat and Subgroup C:solvent. After complete removal of filling materials, digital radiograph was taken. Twenty teeth were selected for applying passive ultrasonic irrigation as an additional adjunct for removing the remaining filling materials. Teeth were then radiographed again, and were re-obturated with gutta-percha and zinc oxide-eugenol-based sealer using lateral condensation technique. The images were submitted to the AutoCAD 2010 software to measure the total canal area and the area of remaining filling material. Then, the ratio of remaining filling material and root canal periphery was computed. Time needed to reach working length was registered. And complications were monitored. Postoperative pain was recorded at the end of retreatment appointment using a visual analog painscale. Patients were instructed to complete a VAS pain score daily for a week. Data was subjected to statistical analysis. p-value < 0.05 was considered significant.Results:No significant differences between the ability of both rotary files (R Endo™,ProTaper®Retreatment) and hand files (GG + H Files + K Flexile Files) regarding to reaching the full root canal working length and the incidence of complications (blockage, fracture of instrument, puff) with or without the aid of heat, solvent or ultrasonic activation (in-vitro & clinically).No significant differences between the ability of heat, solvent and ultrasonic activation whether they were used or not regarding to reaching الملخَّص viiithe full root canal working length and the incidence of complications (blockage, fracture of instrument, puff) whatever the used files were (in-vitro & clinically).Both Rotary files (R Endo™, ProTaper®Retreatment) were significantly faster than hand files (GG + H Files + K Flexile Files) in reaching full root canal working length with or without the aid of heat, solvent or ultrasonic activation (in-vitro), and without any aid (clinically). The time required to reach full root canal working length was significantly shorter when heat, solvent or ultrasonic activation were combined with hand files (GG + H Files + K Flexile Files), whereas these differences did not show with both rotary files (R Endo™, ProTaper®Retreatment) (in-vitro & clinically). Both rotary files (R Endo™, ProTaper®Retreatment) produced significantly cleaner root canals than hand files (GG + H Files + K Flexile Files) whether any adjunct associated to files was not used (in-vitro & clinically).Canal cleanliness was significantly superior when ultrasonic activation was combined with both rotary files (R Endo™, ProTaper®Retreatment) or hand files (GG + H Files + K Flexile Files) compared to heat or solvent (in-vitro).Canal cleanliness was significantly superior when passive ultrasonic irrigation was applied (clinically).Hand files (GG + H Files + K Flexile Files) extruded significantly more filling materials than both rotary files (R Endo™, ProTaper®Retreatment) with or without the aid of heat or solvent or ultrasonic activation (in-vitro). No significant differences between heat, solvent and ultrasonic activation whether they were used or not regarding the amount of apically extruded filling materials when they combined with both rotary files (R Endo™, ProTaper®Retreatment), while solvent significantly decreased the amount of apically extruded filling materials when it combined with hand files (GG + H Files + K Flexile Files) (in-vitro).There is a strong reverse relation between the level of canal wall cleanliness and the amount of apically extruded filling materials. The better files clean, the less files extrude.Levels of postoperative pain during first three days with both rotary files (R Endo™, ProTaper®Retreatment) were significantly less than hand files (GG + H Files + K Flexile Files).Heat increased significantly levels of postoperative pain from first to sixth day, while solvent increased significantly levels of postoperative pain from first to fourth day, heat caused significantly higher levels of postoperative pain than solvent from third to sixth day with both rotary files (R Endo™, ProTaper®Retreatment). ix Levels of postoperative pain after retreatment appointment with teeth which had puff were higher than teeth which had not puff (clinically).Conclusion:According to these results,a-Effectiveness:The ability of reaching working length was equal between rotary files and hand files, and also whether any adjunct associated to files was used or not.Rotary files decreased the duration of retreatment appointment by rapid gain to working length, but the adjuncts associated to files did not.Rotary files produced cleaner root canals than hand files whether any adjunct associated to files was not used.Ultrasonic activation was the best for removing residual filling material, whereas heat and solvent were the worst.b-Safety:Rotary files extruded less filling material and caused less postoperative pain during first three days than hand files.Solvent decreased apically extruded filling material just with hand files, but other adjuncts were passive regarding apical extrusion.Heat and solvent combined with files caused higher levels and longer period of postoperative pain compared to applying files alone.Incidence of complications was equal between hand and rotary files, and also whether any adjunct associated to files was used or not.Postoperative pain after retreatment appointment was related to puff ntroduction:Le principal facteurassocié à l'échec endodontique est la persistance de l’infection microbienne dans le canal radiculaire et /ou dans les zones péri-radicalaire. Il est important de déposer autant que possible des ciments canalaireset gutta-percha lors du retraitement afin d’enlever les tissus nécrosés dans lesquels les bactéries pourraient être la source antigénique.Objectifs de l’étude:Évaluation de l'efficacité et de la sécurité des méthodes manuelles et deux systèmes de rotation continue (R Endo™, ProTaper®Universal Retraitement) pour l’élimination des matériaux intracanalaires.Évaluation de l'efficacité et la sécurité d'utilisation de solvants, la chaleur et l’ultrasons avec le system de retraitement manuel (Gates-Glidden + H limes + K Flexile limes).L'évaluation de l'efficacité et la sécurité d'utilisation de solvants, la chaleur et l’ultrasons avec les deux systems de rotationcontinue (R Endo™, ProTaper®Universal Retraitement).Matériels & Méthodes:Cette recherche se constitue d'une étude in vitro et une étude clinique.Dans une étude in vitro:un total de 144dents mono radiculaire extraites,ont été découronnélaissant une racine d’une longueur moyenne de (21 ± 0,5 ) mm. Lesdents étaient ensuite instrumentésa l’aide d’instruments rotatifs 0,06 taper K3 jusqu'à ISO taille #35, les canaux ont été obturés avec les cônes de gutta-percha et SEALITE™ciment ordinaire utilisant la technique de condensation latérale. Les dents sont alors stockés à 37°C à 100% d'humidité relative pendant quatre semaines pour laisser terminer la prise du ciment. Les dents ont été divisés en trois groupes principaux de 48 dents chacun, avec le Groupe 1:les échantillons ont été soumis à R rotatif Endo™limes et Groupe 2:les échantillons ont été soumis à ProTaper®Universal Retraitement limes rotatives et Groupe 3:les échantillons ont été soumis à (Gates-Glidden + الملخَّص xiH limes + K Flexile limes). Les principaux groupes ont été divisés en quatre sous-groupes (A, B, C et D) de 12 dents chacun selon le facteur associé aux limes utilisées pour l'élimination des matériaux intracanalaires: Sous-groupe A:rien, Sous-groupe B:la chaleur, Sous-groupe C:solvant etSous-groupe D:activation à l’ultrason. pendant la procédure du traitement , les dents étaient attachés à un modèle spécialement conçu pour cette étude permetde fixer les dents et de recueillir les matériaux d’obturation extrudée du canal, qui ont été pesés plus tard à 10-4de précision en utilisant une micro balance. Le temps requis pour atteindre le total de la longueur du travail a été enregistré. Ensuite, les dents étaient longitudinalement divisés en deux moitiés et photographiés. La Propreté desparoisdu canal a été évaluée selon l’échelle de Hulsmann. Les complications ont été aussi notées.Étude clinique:Cette étude a été effectuée sur 135dents mono-radiculaire (antérieures et prémolaires) de 96 patients (28 hommes et 68 femmes) d'âges variant entre 20 et 56 ans, avec une moyenne d'âge de 38,7 ans. L'étude a été limitée aux patients asymptomatiques avec des dents qui sont indiquées pour le retraitement canalaire . Les dents ont été réparties de manière aléatoire en trois groupes principaux de 45 dents chacun selon les limes utilisées pour enleverles matériaux de remplissage: Groupe 1:R rotatif Endo™file, Groupe 2:ProTaper®Universal Retraitement limes rotatives etGroupe 3:(Gates-Glidden + H limes + K Flexile limes). Chaque groupe principal a été divisé en trois sous-groupes de 15 dents chacun, en fonction de l'accessoire qui, combine leslimespour enlever les matériaux de remplissage: Sous-groupe A:rien, Sous-groupe B:chaleur Sous-groupe C:solvant. Après une dépose complète de matériaux de remplissage, une radiographie numérique a été prise. Vingt dents ont été sélectionnées pour l'application passive de l'irrigationà l’ultrason comme un complément additionnel pour retirer les autres matériaux de remplissage. Les dents étaient ensuite radiographiées à nouveau, et ont été rebouchées avec les cônes de gutta-percha, et le cimentoxyde de zinc et eugénolavec la technique de condensation latérale. Les images ont été soumisesaulogiciel l'AutoCAD 2010 pour mesurer la zone total du canal et la zone de remplissage des matériaux restants. Ensuite, le rapport entre les matériaux restants de remplissage et la périphérie canalaire a été calculé. Le temps nécessaire pour parvenir à la longueur du travail a été enregistré. Et les complications étaient surveillées. La douleur postopératoire a été enregistrée à la fin de laséance de retraitement en utilisant un échellevisuel analogique de douleur. Nous demandons aux patients de remplir un SAV score de douleur quotidiennement pendant une semaine. Les données ont été soumises à l'analyse statistique. La valeur p < 0,05 a été considérée comme significative. الملخَّص xiiRésultats:il n’y a pas de différence significative entre la capacité les deux limes rotatif (R Endo™, ProTaper®Retraitement) et les limes manuelles (GG + H limes + K Flexile limes) pour atteindre letotalde la longueur du travail du canal avec ou sans l'aide de la chaleur, du solvant et de l’activation à l’ultrason (in-vitro & cliniquement).il n’y a pas de différence significative entre la capacité de la chaleur,du solvant et de l’activation à l’ultrason si elles ont été utilisées ou non pour atteindre la totalde la longueur du travail du canal quelles que soient les limes utilisées (in-vitro & cliniquement).Les deux limes rotatif (R Endo™, ProTaper®Retraitement) étaient considérablement plus rapides que les limes manuelles (GG + H limes + K Flexile limes) en atteignant la totalde la longueur du travail du canal avec ou sans l'aide de la chaleur ou du solvant ou à de l’activation à l’ultrason (in-vitro), et sansaucune aide (cliniquement).Le temps requis pour atteindre letotalde la longueur du travail du canal était significativement plus court lorsque la chaleur ou un solvant oul'activation à l’ultrason sont combinés avec les limes manuelles (GG + H limes + K Flexile limes), tandis que ces différences n'étaient pas visibles avec les deux limes rotatif (R Endo™, ProTaper®Retraitement) (in-vitro & cliniquement).Les deux limes rotatif (R Endo™, ProTaper®Retraitement)) produisent un canal significativement propre par rapport les limes manuelles (GG + H limes + K Flexile limes) quelque soit le complément utilisé (in-vitro & cliniquement).La propreté du canal était significativement supérieure à l’ultrason lorsque l'activation a été combinée avec les deux limes rotatif (R Endo™, ProTaper®Retraitement) ou avec les limes manuelles (GG + H limes + K Flexile limes) par rapport à la chaleur ou le solvant (in-vitro).la propreté du canal a été significativement supérieur lorsque l'irrigation par l’ultrason passive a étéappliquée (cliniquement).Les limes manuelles (GG + H limes + K Flexile limes) ont extrudé sensiblement plus de matériaux de remplissage par rapport aux deux limes rotatif (R Endo™, ProTaper®Retraitement) avec ou sans l'aide de la chaleur ou du solvant ou de l'activation à l’ultrason (in-vitro).il n’y a pas de différence significative entre la chaleur et le solvant et l'activation à l’ultrason si ils ont été utilisés ou non,en ce qui concerne le taux de matériaux de remplissageapicalement extrudés quand ils ont été combinés avec les deux limes rotatif (R Endo™, ProTaper®Retraitement), tandis que le solvant a sensiblement diminué le taux de matériaux de الملخَّص xiiiremplissage apicalement extrudé lorsque il est combiné aux limes manuelles (GG + H limes + K Flexile limes) (in vitro).Il existe une forte relation inverse entre le niveau de la propreté de la mur de canal et la quantité des matériaux de remplissageLe niveaude la douleur postopératoire pendant trois jours avec les deux limes rotatif (R Endo™, ProTaper®Retraitement) était sensiblement inférieurcontrairement avecles limes manuelles (GG + H limes + K Flexile limes), alors que ces différences n'étaient pas notifiables entre les deux limes rotatif (R Endo™, ProTaper®Retraitement).La chaleur a augmenté considérablement les taux de la douleur postopératoire entre la première et la sixième journée, tandis que le solvant a augmenté considérablement les taux de la douleur postopératoire entre la première à la quatrième journée, la chaleur cause considérablement plus de douleur postopératoire que le solvant entre la troisième à la sixième journée avec les deux limes rotatif (R Endo™, ProTaper®Retraitement).Le niveau de la douleur postopératoire lorsdu prochain rendez-vous de traitement avec des dents qui avaientun dépassement de ciment au-delà du periapex était plus élevé que avec les dents qui n'avaient pas ce dépassement (cliniquement)Conclusion:Selon ces résultats a-L’efficacité :la capacité de parvenir à la longueur du travail était égale entre le limes rotatifs et les limes manuelles, et de meme si un complément associé aux limes a été utilisé ou non.Les limes rotatifs ont diminué la durée de rendez-vousde retraitement grace au gain de temps pour arriver à la longueur du travail, mais les annexes associées aux limesn’en tiennent pas compte.les limes rotatifs produisent des canaux plus propre par rapport les limes manuelles si aucun complément associé n'était pas utilisé.L’activation àl’ultrasons a été la meilleure méthode pour supprimer les résidus de matériau, tandis que la chaleur et le solvant étaient moins efficaces.b-Sécurité :Les limes rotatif extrudentmoins de matériaux de remplissageet causent moinsde douleur postopératoire pendant trois jours que avec les limes manuellesLe solvant diminue l’extrusion apicalde matériau de remplissage seulement avec les limes manuelles, mais d'autres systèmes annexes ont été passifsen ce que concernel’extrusion apicale. xiv La chaleur et le solvant combinés à des limescausent un niveau plus élevé et une période plus longue de la douleur postopératoire par rapport à l'application des limes seules. L’Incidence des complications était égale entre les limes manuelles et rotatif, et également si un complément associé aux limes a été utilisé ou non. La douleur postopératoire après le prochain rendez-vous de traitement était liéeau ciment dépassé apicalement.
