الملخص

Type1 Diabetes Mellitus (T1DM) is a disorder of glucose homeostasis characterized by autoimmune destruction of the insulin producing pancreatic β cells which progressively leads to insulin deficiency and resultant hyperglycemia. The aim of the present study was to evaluate the management of Type 1 diabetic patients at Wad- Medani Paediatric Teaching Hospital, Sudan. This study was a retrospective cross sectional hospital based study, composed of two phases, in phase one special designed form was used to obtain data from regular follow up sheet documents in the hospital files, while phase two was a structured interview with the multi-disciplinary team to know the reasons of uncontrolled blood glucose concentration in Type 1 diabetes and the solutions. As a base line a data investigations of 100 Type 1 diabetic patients were taken including glycosylated hemoglobin (HBA1c) and fasting blood glucose ( FBG) accompanied by health education program about diet, physical exercises, symptoms of hypoglycemia, and Diabetic Keto Acidosis ( DKA). Before six months, three months and one month from the beginning of the study. The obtained results revealed that there was a significant (p≤ 0.001) progressive improvement in the glycemic control of Type 1 diabetes mellitus reflected as, glycosylated hemoglobin (HBA1c) before six months there were 6 (6%) patients their Type 1 diabetes was controlled while 94 (94%) patients were uncontrolled. Also there were 30 (30%) and 70(70%) patients their diabetes was controlled and uncontrolled respectively before three months. Before one month it was found that Type 1 diabetes was controlled in 49 (49%) patients and uncontrolled in 51 (51%) patients. Goiter was found to be the most predominant disease among Type 1 diabetic patients; before six months (51%), before three months (49%) and before one month (35%). It could be concluded that health education program (diet control, exercise) and regular follow up were the most efficient methods for glycemic control of Type 1 diabetic patients. . It could be recommended that national diabetes education program should be incorporated in different facilities and the pharmacist should be an active participant to follow structured program to identify and manage drug related problems.
