The Effect of Anthypertensive Drugs Combined Therapy on Plasma Glucose and lipid profile among the adult Hypertensive patients

Antihypertensive drugs are widely used for treatment of hypertension, but thiazide diuretics and β-blockers tend to be associated with metabolic adverse effects. The purpose of this prospective study was to investigate the effect of different antihypertensive combined therapy on plasma glucose and lipid profile in adult hypertensive patients and to compare the efficacy of these different antihypertensive combined therapies on blood pressure levels. To achieve this, 60 newly diagnosed hypertensive patients were selected from UNRWA clinics in Gaza strip- Palestine and were divided into two groups (G1 and G2 ). The first group (n=30) was treated with Atenolol (25-50mg/day) and Enalapril (10-20mg/day) combined therapy whereas the second group(n =30) was treated with Hydrochlorothiazide (12.5-25mg/day), Atenolol (25-50mg/day) and Enalapril (10-20mg/day) combined therapy. Plasma glucose, triglycerides (TG), total cholesterol (TC), high density lipoprotein (HDL-c) and low density lipoprotein (LDL-c) levels were measured before starting treatment and then at 3, 6 and 9 months of the study period. Also blood pressure were measured before and at 3, 6 and 9 months of the treatment. The results showed a significant increase in the plasma levels of triglycerides in the first and the second group of hypertensive patients at 6 and 9 months of treatment when compared with the baseline values. Also the results showed a significant increase in plasma levels of total cholesterol and LDL-c in the first group at 6 and 9 months while in the second group at 3, 6 and 9 months when compared with the baseline values. There were no significant differences in the plasma glucose and HDL-c levels throughout the study period. The study data suggest that combined therapy with Atenolol and Enalapril have less metabolic adverse effects than the combined therapy with Hydrochlorothiazide, Atenolol and Enalapril in adult hypertensive patients throughout the nine months of the study period. The results also showed that combined therapy was effective in lowering blood pressure in G1 and G2 hypertensive patients throughout the treatment period.
