- This study tackles an important issue in public health; it is the psychologicalproblems ofpatients receiving primary services at the clinics of thePalestinian Ministry of Health in the city of Hebron. The study was conductedover a period of one month between 01-09-2007 and 30-09-2007, during which fivepublic clinics out of about 118located in different parts of the city weresurveyed.The researcher chose this sample only for reasons related to limitedhuman resources and finances.The studyaimed at identifying the rates of psychological disorder in patientsattending the clinics and the relationship between these disorders anddemographic and other independent factors,. The researcher used the Ex PostFacto method and the sample in this study were all the patients (aged between 18and 59 years, females and males)who attended the five clinics and who gavetheir consent to be part of this research. The researcher visited clinics on 25occasions in September. It is worth mentioning that all the patients werecovered by health insurance (80% of the population are covered). Other patientswho presented at the clinics for referrals, vaccinations, insurance, medicalclearance, prenatal check-ups, pregnancy-related problems and emergency caseswereexcluded. Four hundred and fifty (450) patients within the target age groupwere requested to fill out a questionnaire, 340 agreed to be included, 4questionnaires were discarded as they lacked basic data. The researcher used aquestionnaire about the social and economic status. which included thevariables: gender, age, education, social status, economic status, occupation,and physical health status. The latter included, type of disease, frequency ofvisits to physician, duration of chief complaint, and use of medications.Another tool called Derogatis Scale SCL-90-R that measures physical symptoms,obsessive-compulsive disorder, interpersonal sensitivity, depression, anxiety,aggressiveness, phobic anxiety, paranoid ideation, psychoticism, was also usedin collecting the data. All the patients took part voluntarily in the study andwere contacted personally or interviewed so as to answer questions over a periodof 10 to 20 minutes. All the data were analyzed through SPSS and different statistical methods, suchas levels of frequency, percentage, mean, standard deviation, multi-regression,co-efficient factor, non-parametric statistical methods, such as U test for ManWhitney and kroscal-Wallis, as the data do not conform to the normal distribution.The results showed that 24.1 % of the sample had psychological symptoms, and 3.3% of which according to Global Indices was severe. Differences in the level ofpsychological disorders were found to be associated with levels ofeducation inall the cases; thehigher the level of education was, the less the frequency ofthe psychological problems and vice versa. Other factors included: low-incomeand duration of chief complaint. 39.3 % showed psycho-somatic signs, 11% ofwhich was severe. 33.3 % had obsessive-compulsive disorders and 8% of which wassevere. As for the interpersonal sensitivity(can you add a comment on what thismeans?) cases, 31.2 % needed intervention and 7.1% needed to be referred formedical treatment.29.1% suffered from depressionand of whom 6.5% were severe. 26.8% suffered from anxiety, and of whom 3.6% wereto be referred for medical treatment. 27.9% suffered with Hostility, and of these 8.9% needed intervention. As for the phobias, 16.1%needed intervention, and of which 1.8% was severe. 30.6% suffered from paranoidideation, and which 7.7% needed medical intervention. 17.9% suffered frompsychoticism, andof whom 2.1% needed medical treatment.The researcher came out with several recommendations, the most important ofwhich are giving more financial support and attention to psychological healthservices so as to meet the public demandwhichneedstreatment, education, andprevention, in addition to channelling the researchshe also recommends thatthey be made available in primary health programs in hospitals, and that the societal and psychological services receive the neededdevelopment through the training of doctors and nurses working in primary healthclinics so that they become able to identify cases that require treatment orreferral to the local CMHC. Finally,first importantshe recommends that more psychotherapistsand social workers be incorporated in public clinics.
