Background Isotretinoin is a form of vitamin A, which is widely used in the treatment of multiple skin conditions such as the Acne vulgaris , Rosacea , seborrheic dermatitis, some of the disorders of keratosis(eg psoriasis, Ichthyosis, etc.) and is used to prevent skin tumors. But it has a wide range of side effects; the most common are skin dryness and mucous membranes .Obgective :Study the side effects of isotretinoin and its intensity according to VAS%, discuss with patients and explain ways of managing them, as well as compare the rate of occurrence of these effects between different doses of isotretinoin and how to develop during the months of treatment with monitoring of laboratory disorders and response to treatment and the rate of recurrence during the follow-up of cyclic patients to investigate any side effect left after the end of the treatment. Material and Methods :This descriptive study was conducted on 768 patients from the Department of Dermatology and Venereal Diseases at Tishreen University Hospital in Lattakia, aged between 15 and 74 years. The side effects and severity were assessed and measured according on a vas% scale monthly during the treatment period .Results: The sample included 768 patients(363 males and 405 females), mean age(24.25), and average weight(63.45). The most common side effects they had were mucosal skin effects that included; Cheilitis 67.3% of patients, then dry skin 36.2%,followed by eye and nose dryness(22%) ,then fatigue and eczema. These side effects were clearly dependent on the dose of isotretinoin, mild-intensity, predictable and gradually decreased with continued treatment. Laboratory abnormalities were rare and transient. A high clinical response was observed with a positive correlation between the dose of isotretinoin and the rate of response to treatment, while a reverse correlation was observed between dose and recurrence rate.CONCLUSION: This study showed that the side effects of isotretinoin treatment were mild, predictable, usually temporary, tolerable and easy to manage without the need to stop medication. Most patients treated were very satisfied with the treatment
