Prevalence, predictors and outcomes of ICU-acquired central line-associated bloodstream infections in Oman

Central line-associated bloodstream infections (CLABSIs) rank among the three most common hospital-acquired infections, particularly in critical care settings. They significantly impact patient outcomes and healthcare systems globally. Extensive research has documented their association with increased mortality, prolonged hospital stays, and elevated healthcare costs. Despite widespread use of central lines in ICUs in Oman, no prior studies have assessed the prevalence, predictors, and outcomes of CLABSIs among adult ICU patients in the country. Aim: This study aims to estimate the prevalence of ICU-acquired CLABSIs, identify the predominant causative microorganisms, determine associated risk factors, and evaluate the mortality rate, length of hospital stay, and additional healthcare costs linked to ICU-acquired CLABSIs among adult ICU patients in Oman. Method: A retrospective case-control study design was employed. A comprehensive electronic medical records review was conducted for all adult ICU admissions over two years (2018–2019) at two tertiary hospitals in Oman. A case group of 58 patients with ICU-acquired CLABSIs was identified and compared to a randomly selected control group of 174 ICU patients without CLABSIs. Results: The prevalence of ICU-acquired CLABSIs was 8.9 and 8.31 per 1,000 catheter-days in 2018 and 2019, respectively. Gram-positive bacteria were the most frequently isolated pathogens (46.6%). Significant risk factors identified by logistic regression included heart failure (OR=11.67, p<0.001), presence of other infections (OR=3.4, p=0.009), hemoglobin level (OR=0.04, p<0.001), tracheostomy (OR=5.34, p=0.004), and blood transfusion (OR=0.168, p=0.001). The mortality rate associated with ICU-acquired CLABSIs was 70.1 deaths per 1,000 adult ICU patients with central venous catheters (CVC), with a case fatality rate of 77.6%. These infections added an average of 17.5 days to hospital stays and increased healthcare costs by an estimated 3,308 Omani Rial per case. Conclusion: The prevalence of ICU-acquired CLABSIs in Oman is substantially higher than reported rates in developed countries. Several modifiable risk factors were identified, highlighting opportunities for targeted interventions. The study also estimated the significant economic burden of these infections, providing a basis for forecasting cost savings following implementation of preventive measures. These findings strongly support adopting a bundle of care interventions in line with updated guidelines from the World Health Organization and CDC to effectively prevent ICU-acquired CLABSIs.
