Introduction: Prolonged treatment duration is considered one of the main disadvantages of orthodontic treatments, that most of these traditional treatments take more than a year to be accomplished. Unfortunately, many patients avoid orthodontic treatment due to its long duration and accompanying pain, so reducing orthodontic treatment time is important for orthodontists and patients especially adults in terms of reducing its disadvantages such as pain and discomfort. Several techniques have been developed, such as Piezocision in attempt to accelerate orthodontic teeth movements .Aim of study :Evaluate the effectiveness of piezocisionin acceleration decrowding of severe lower anterior teeth and patient-centered outcomes. Materials and Methods: A Parallel-group randomized controlled trial was conducted on patients with severe mandibular irregularity. Sample size was 34patients (19females and 15males), then subjects were divided into two parallel groups using Minitab® Version 17ّsoftware. First group received conventional orthodontic treatment, whereas the second group received piezo-assisted orthodontic one .Little`s irregularity index changes were studied using dental cast in four time points before inserting the first arch wire (T0), after 1month of treatment onset (T1), after 2 months (T2), and at the end of the alignment stage(T3).Visual Analog Scale (VAS) questionnaires were used in order to evaluate the levels of patients` acceptance, pain and discomfort. Subjects received their questionnaires at the following assessment times: a day after orthodontic treatment onset, a week, 2 weeks, a month. Results :The overall treatment time needed to complete anterior alignment of the mandibular dental arch was reduced (P<0.0001)by(59%).Patients in piezocision group showed significantly higher levels swelling compared with the control group only a day after procedure (P=0.011)and there was no statistically significant difference between two groups during the rest of evaluation times. No statistically significant differences between the two groups were detected regarding levels of pain, discomfort, difficulties of mastication, swallowing and jaws movement limitation through 89the different assessment times( P<0.05),as that experimental subjects` acceptance mean accompanying the acceleration technique was high that reached (86.47%).Conclusions: Minimally invasive Piezocision accelerated orthodontic decrowding significantly by (595), and achieved a high level of acceptance and satisfaction one month postoperatively and raised a low amount of pain , discomfort ,swelling, difficulties of mastication, swallowing and jaws movement limitation level
