Evaluation et comparaison des différents protocoles thérapeutique contre le SARS-COV-2 et leur efficacité antivirale à Médéa

الملخص
La COVID-19 est une nouvelle maladie causée par le virus SARS-COV-2. L’Algérie a adopté différents protocoles thérapeutiques. L’objectif de cette étude est d’évaluer est comparé ces protocoles afin d’arriver au protocole le plus efficace.Pour cela une étude rétrospective descriptive a été accomplie, des données ont été recueillies auprès des patients admis à l’hôpital Mohamed Boudiaf à Médéa avec RT-PCR positive pour le SARS-COV-2de mars et à décembre 2020. Les données ont été analysées avec le logiciel Excel. Le test khi-deux a été utilisé pour comparer les variables catégorielles. Les valeurs P 0,05 ont été considérées comme statistiquement significatives.Cette étude a montré que l’hydroxychloroquine a un effet perturbateur par rapport au niveau des leucocytes et la glycémie et augmente le D-dimere, cependant l’anticoagulant a montré un effet d’arranger le niveau de D-dimere et perturbation de glycémie. Pour le diffèrent protocole, protocole anticoagulant-curatif 33.68% ont été réanimé et odd ratio 0.16 (p-value <0.001) ; l’évolution 96.84% guéris ; 3.15% décédés et odd ration de 0.31 (p-value <0.05). Protocole hydroxychloroquine/anticoagulant-préventif 85.49% ont été réanimé et odd ratio 5.59 (p-value <0.001) ; l’évolution 92.74% guéris ; 7.25% décédés et odd ration de 0.8 (p-value <0.5). Protocole corticothérapie/anticoagulant-curatif 25% de réanimation et odd ratio 0.15 (p-value <0.001) ; l’évolution 96.84% guéris ; 3.15% décédés et odd ratio de 0.15 (p-value <0.9).Le protocole anticoagulant-curatif à montrer plus d’efficacité par rapport aux autres protocoles due aux effets secondaires de l’hydroxychloroquine et corticoïdes. Le protocole corticothérapie/anticoagulant-curatif est prometeux mais nécessite plus étude.

- After the recovery from COVID-19,long-term consequences were diagnosed. The objective of this work is to target these after-effects through an investigation in the city of Medea at Mohamed Boudiaf’s hospital.A study was carried out in the prevention service on a sample of 428 patients between March and December 2020. Informations were collected for each patient by filling out a survey that we established and which contains the general characteristics and data of the patients infected.On a sample of 428 patients; 189 have developed post-COVID evolutions. The percentage of respiratory involvement was estimated at 15.2% that of cardiac involvement was 6.9% in 2nd position; as well as for diabetic disease which also affects 6.9%. For all the patients who presented renal impairment which remains an infrequent course, the percentage was 2.7%. For the other possible sequeals, the percentage was 11.6%. Finally, the case fatality rate was 7.4%.Age and progression of the infection have been found to be closely related according to the KHI-DEUX test. Therefore in this study; it can be concluded that age is an important risk factor for the progression of COVID-19
