Acute bacterial meningitis is still a very common and serious disease particularly in the neonatal period than any other time in life and it is an important cause of morbidity and mortality globally. Acute bacterial meningitis cannot be diagnosed on the basis of clinical signs and symptoms, So rapid examination of CSF is considered as an essential step in early diagnosis. The study was included 50 CSF samples taken from neonates, Infants and children below the age of 5 years, who were clinically suspected of Acute bacterial meningitis and visiting the Department of Paediatric in Aleppo University hospital during the period from 5/5/2014to 10/11/2014.Laboratory investigations included Gram’s stain, bacteriological culture test, biochemical tests and API (Analytical profile index) system to isolate and identify the bacterial causes of meningitis from CSF. The organisms isolated in the study were characterized by antibiotic susceptibility tests by disc diffusion method. Bacterial pathogens were isolated from 28 samples showing an isolation rate of 56% by culture test. Eight bacterial isolates, one isolate of Escherichia coli, four isolates of Neisseria meningitides and three isolates of Staphylococcus aureus were confirmed by using API ID 32Esystem,API NH system and API STAPH system respectively. Among 50 cases who were clinically suspected acute bacterial meningitis, Our study was showed that gram negative bacteria were the most common isolates (78.57%) while (21.43%) were gram positive bacteria. Pseudomonas aeruginosa were the most commonly isolated bacteria among the bacterial isolates (21.4%), followed by K. pneumoniae )14.3%(,Neisseria meningitides )14.3%(,Acinetobacter spp. )10.7%(, Staphylococci aureus )10.7%(,gram positive bacilii )7.1%(,Proteus spp. )7.1%(, E.coli )3.6%(, Streptococcus pneumoniae )3.6%(, Haemophilus influenzae)3.6% (and Enterobacter spp. )3.6%(.this study showed an increase in the ratio of Gram negative Bacilli resistance to different types of used antibiotics, especially the third and fourth generation of cephalosporins and were resistance to specific antibiotics such as Meropenem, 133wherease Gram Positive Bacteria were resistance to most antibiotics including Ampicillin, and Penicillin but showed high sensetive to Vancomycin. The recent research showed that the rate of occurrence of acute bacterial meningitis in the neonates of the age between 1–20 days was (46.43%)and it was significantly )p=0.024.(where ase, the rate of occurrence of acute bacterial meningitis in the children of the age between 6 months to 5 years was (10.34%). So it is important to examine the neonatal period care fully to prevent the disease from distributioning. There is a need for periodic review of bacterial meningitis worldwide, since the pathogens responsible for infection vary with geography and time, So This research demonstrates that it is important to have a good understanding of the epidemiology of acute bacterial meningitis in one’s own region. This too, is true for the patterns of antibiotic sensitivities. Knowing the most common causes of bacterial meningitis helps health professionals to initiate the best possible empiric antibiotic therapy
