Les Aspects neurochirurgicaux des adénomes hypophysaires: expérience du service de neurochirurgie CHU Mohamed VI de Marrakech

الملخص

Les adénomes hypophysaires, tumeurs bénignes se développent aux dépens des cellules du lobe antérieur de l’hypophyse, et constituent 10 % des tumeurs cérébrales. Nous rapportons une étude rétrospective concernant 32 cas d’adénomes hypophysaires colligés au service de neurochirurgie du CHU Mohamed VI de Marrakech entre janvier 2002 et décembre 2010. L’âge moyen de survenue était de 37,63 ans avec une nette prédominance féminine (59,37 %).Il relève de notre étude un retard diagnostique assez long avec une moyenne de 3 ans, d’où la fréquence élevée des troubles visuels (56 ,25%), des signes d’HIC (81,25 %) ainsi que des troubles endocriniens (62,50 %).Le diagnostic est orienté par la radiographie du crâne centrée sur la selle turcique dans 10 cas, affirmé par la TDM et/ou IRM ainsi que les dosages hormonaux et confirmé par l’histologie. Au plan thérapeutique, La voie trans-sphénoïdale a été utilisée dans 68,75 % des cas, tandis que 12,5 % des cas ont été abordés par voie endoscopique, la voie trans-crânienne est pratiquée chez 18,75 % des cas. Le traitement médical à base de bromocriptine a été préconisé chez 6 patients, dont 5 en association avec la chirurgie. Deux patients ont bénéficié de la radiothérapie postopératoire. La guérison clinique et radiologique est obtenue chez 7 patients (35%), l’échec chirurgical est constaté dans 4 cas et la récidive dans 3 cas (15%).aucun décès n’a été signalé, la morbidité est faible et la mortalité est exceptionnelle si les indications sont respectées. La revue de la littérature montre que ces tumeurs atteignent surtout le sexe féminin(entre 52 et 92%),l’âge moyen varie entre 31 et 47 ans, la fréquence du syndrome d’HTIC varie entre 28 et 90%,celle des troubles visuels entre 54,5% et 83,3%, les signes endocriniens varient selon le type d’adénome .la voie transsphénoïdale reste la méthode de choix puisqu’elle est utilisée entre 95 et 100% des cas selon la littérature, l’utilisation du traitement médical varie selon le type d’adénome hypophysaire.

The hypophyseal adenomas are known as benign tumors, and to the detriment of the cells of the anterior lobe of the hypophysis and represent 10% of the cerebral tumors. We report a retrospective study about 32 cases oh hypophyseal adenomas, collected at the department of neurosurgery in UHC Mohamed VI in Marrakech, between January 2OO2 and December 2010.The mean of age of supervere was about 37,63% with meat female predominance(59,37 %).In our study, we notice a quite long diagnostic delay that the average is 3 years, tence the high frequency of visual disorders (56 ,25 %), signs of intra cranial high blood pressure (81,25 %), the endocrinal signs((((62,50 %).).).).The diagnostic is oriented by the radiography of the skull centred on the pituitary fossa in 10 cases, clained by the computerized tomography and/or the IRM as well as the hormonal dosages and confirmed by the histology. About the therapeutics, The transphenoid passage has been used in 68,75% of the cases, wheras 18,75 % of the patients has been approach by transcranial way, 12,5% the cases has been approach by endoscopy.... The medical treatment based on the bromocriptine has been recommended in 6 patients, that 5 are associated to the surgery. Two patients benefitted from radiotherapy after the operation. The clinical and radiological recovery is achieved in 7 patients (35%), the surgical unsuccess is noticed in 4 cases and the recidivism in 3 cases(15%).there was no death. The morbidity is low and the mortality is exceptional if the indications are respected. The review of the medical literature shows that these tumors reach particularly the female sex(between 52 and 91%),the mean age is from 31 to 47 years old, the frequency of the ICHBP syndrome is between 28 and 90%,this of the visual confusions is between 54,4% and 83,3%,the endocrinal signs varie according the type of the adenoma. The transsphenoidal approach is still the choice method as it is used between 95 and 100% of the cases according the literature, the use of the medical treatment varies according the type of the hypophyseal adenoma.
