Épidémiologie des onychomycoses à l'Hôpital Militaire Avicenne de Marrakech: Expérience du service de Parasitologie et Mycologie Médicale

الملخص
L’onychomycose est définie comme une infection fongique de l’ongle provoquée par des champignons. Cette affection n’est pas considérée uniquement comme un simple problème esthétique mais aussi comme un handicap physique, fonctionnel et psychique. Elle peut constituer un point de départ d’une infection grave chez les immunodéprimés. Nous avons mené une étude rétrospective descriptive analytique, sur 5 ans (1er Janvier 2008 - 31 Décembre 2012) réalisée au sein du service de Parasitologie Mycologie à l’Hôpital Militaire Avicenne de Marrakech. Parmi 1044 patients qui sont adressés au laboratoire pour suspicion clinique d’onychomycose afin de réaliser une étude mycologique, 671 cas ont eu une onychomycose confirmée, soit une prévalence de 64.3%. Le sexe ratio H/F était de 0.87, dont 5,52% étaient hospitalisés. Pour les facteurs favorisants, le port des brodequins (38% de militaires) aussi le manque de séchage des orteils après les ablutions dans notre pratique religieuse constituaient un microclimat ; associant la chaleur et l’humidité ; essentiel pour le développement des champignons. Durant notre étude, l’atteinte des ongles des orteils était la plus fréquente avec 75% des cas. Le type de lésion associée le plus fréquemment, était l’intertrigo interorteil avec 18% des cas, contre seulement 1% des cas ayant eu un intertrigo interdigital associé à l’onychomycose. Le champignon le plus impliqué était le Trichophyton rubrum (74.3 %),suivi du Candida albicans (24.6%)puis Trichophyton interdigitale (0.73%), et en dernier venaitScopulariopsis brevicaulis et Scytallidium hyalinum. La confirmation mycologique de l’étiologie fongique de l’onychopathie et la précision de l’espèce incriminée sont des éléments décisifs pour le choix d’un traitement approprié. Ce qui donne au laboratoire une place très importante dans le diagnostic mycologique des onychomycoses, constituant d’une part un diagnostic différentiel avec de nombreuses dermatoses d’expression clinique similaire et d’autre part, exigeant une prise en charge particulière.

- Onychomycosis is a mycose of nail due to fungi. This affection is not limited only to the aesthetic appearance but They may have an important the physical, functional, psychological handicap and can be the starting point of serious infections in immunocompromised. We conducted retrospective descriptive study of 5years (1 January 2008 - 31 December 2012 ) at the Parasitology and Mycology laboratory at the Military Hospital of Marrakech. During the study period, 1044 patients were included with a total of 671 identified onychomycosis, with prevalency of 64.3%. The sex ratio M / F is 0.87, which 5.52% were hospitalized. For predisposing factors, wearing boots (38% military) also the lack of drying toes after ablutions in our religious practice are a microclimate; combining heat and moisture; essential for the growth of fungs. In our study, the achievement of toenails is the most common with 75% of cases. The fingers of the hand, with a prevalence of 76% of lévurique origin. The type of lesion associated most frequently is intertrigo interorteils with 18% of cases, against only 1% of cases with an interdigital intertrigo associated with onychomycosis. The more involved the fungus Trichophyton rubrum, (74.3 %), followed by Candida albicans (24.6%) and Trichophyton interdigitale(0.73%), Scopulariopsis brevicaulis and Scytallidium hyalinum. The Mycological confirmation of fungal etiology of onychopathy and accuracy of the offending species are decisive for the choice of appropriate treatment. Which gives the laboratory a very important place in the mycological diagnosis of onychomycosis, constituting both a differential diagnosis with many dermatoses similar clinical expression and secondly, requiring special care.
