الملخص

ABSTRACTIntroduction: Birth defects (congenital anomalies) are the leading cause of death in babies under 1 year of age; the most common disorders include congenital heart defects, neural tube defects and Down syndrome. Neural tube defects are the second most common congenital conditions affecting the nervous system. Globally 500,000 children are born each year with neural tube defects. The highest incidence reported was in a rural area in northern China where the incidence was 6/1000 before the supplementation of women in the child bearing age with folic acid when the incidence dropped to 2/1000. In Sudan the incidence of all congenital anomalies including NTDs was 5.82/1000, the incidence of NTDs was 3.48/1000, thus: NTDs constituted 59.4% of all congenital anomalies at birth_ highest incidence in Africa.Objectives: the study aimed to determine types of neural tube defects, to estimate the different patterns according to the geographic distribution, to assess the associated anomalies and to assess the associations of the neural tube defects with: Prenatal folic acid supplementations, maternal diseases and Drugs.Methodology: Descriptive cross sectional study design in Khartoum state hospitals. Sample size was 30 patients as total coverage of all cases in the period of the study.Result: The study revealed that male Infants are most common affected by neural tube defects (60.0 %), while (40.0 %) of infants are females (male: female ratio 3:2). (66.7%) of mothers were between 25-35 years old and (50%) of them have No folic acid supplementations, (33.3%) of them consume it in first trimester, (10%) in third trimester and (6.7%) in Second trimester only (20%) of mother were taking antiemetic's and (16.7%) were taking antibiotics in the first trimester. The study found that the commonest type of neural tube defects was Meningomylocele (53.3.0%) followed by Meningocele (40.0%) and (6.7) had encephalocele. Neural tube defects were found to be (53.3%) in Lumbar region, (16.7%) sacral region and (10%) in thoracic region. The study also revealed that (30%) of infants with neural tube defects had Hydrocephalus, (7%) have limbs deformity and (3%) had Cardiac anomalies. The study showed a significant relation between the Type of neural tube defects and lower extermity weakness (P value =0.028-(table 9)) This strong relation is due to that the meningomylocele sac contains either part of the spinal cord or spinal nerve root within it. Also a strong significant relation between maternal drugs during the first trimester and the type of neural tube defects P value = 0.016 (Table 8)., this means that the drug is a risk factor to the neural tube defectsConclusion: the study concluded that some drugs as causing NTDs and the Neural tube defects causing paralysis to infants. More researches are needed to estimate its prevalence and more wide survey of its patterns and risk factors to form strong base for application of preventive policies and programs such as fortification with folic acid and support system for the affected families.Recommendations: Using through the mass media and other channels information regarding the neural tube defects, the risk factors, and the importance of folic acid consumption should be forwarded to mothers. The establishment of programs to prevent and control this condition is necessary, as improving the diet of women throughout their reproductive years, ensuring an adequate dietary intake of vitamins and minerals such as folic acid and iodine.Psychological rehabilitation for parents to reduce the impacts of NTDs on their families and to improve their infant life style.More research using more samples is recommended for a better outcome and better life..

تشكل العيوب الخلقية (التشوهات الخلقية) السبب الرئيسي للوفاة لدى الأطفال دون سن سنة؛ وتشمل العيوب الأكثر شيوعا إختلالات القلب الخلقية، عيوب الأنبوب العصبي ومتلازمة داون. عيوب الأنبوب العصبي هي النوع الثاني الأكثر شيوعاً.هي من العيوب الخلقية التي تؤثر على الجهاز العصبي. عالميا يولد 500,000 طفل كل عام بعيوب الأنبوب العصبي. وكانت أعلى نسبة أبلغ عنها في منطقة ريفية في شمال الصين حيث كان معدل الإصابة6/1000قبل تعويض النساء في سن الإنجاب بحمض الفوليك عندها انخفض معدل الإصابة إلى2/1000.الأهداف: هدفت الدراسة إلى تحديد أنواع عيوب الأنبوب العصبي وتقدير الأنماط المختلفة وفقا للتوزيع الجغرافي وتقييم الشذوذات المرتبطة بها وتقييم ارتباطات عيوب الأنبوب العصبي مع: مكملات حمض الفوليك قبل الولادة وأمراض الأمهات والأدوية التي تتناولها الامهات خلال الثلاثة اشهر الاولى من الحمل.المنهجية: دراسة وصفية مستعرضة اجريت في مستشفيات الخرطوم. وكان حجم العينة 30 مريضا كما التغطية الإجمالية لجميع الحالات في فترة الدراسة.النتيجة: كشفت الدراسة أن الذكور الرضع هم الأكثر تأثرا من عيوب الأنبوب العصبي (60٪)، في حين أن (40٪) من الرضع هم من الإناث (نسبة الذكور: الإناث 3: 2). (66,7٪) من الأمهات تتراوح أعمارهن بين 25-35 سنة و (50٪) منهن لم ياخذن مكملات حمض الفوليك خلال فترة الحمل، (33.3٪) استهلكنه في الأشهر الثلاثة الأولى، و 10٪ في الثلث الثالث و 6.7٪ في الثلث الثاني. فقط (20٪) من الأمهات أخذن مضادات الأكسدة و (16.7٪) كانوا يتناولون المضادات الحيوية في الأشهر الثلاثة الأولى. ووجدت الدراسة أن النوع الأكثر شيوعا من عيوب الأنبوب العصبي هو مينينغوميلوسيل (53.3.0٪) تليها مينينغوسيل (40.0٪) و (6.7) يكون الدماغية.الموقع الأكثر شيوعا من عيوب الأنبوب العصبي هو المنطقة القطنية (53.3٪)، المنطقة العجزية (16.7٪ ) و (10٪) في المنطقة الصدرية. وكشفت الدراسة أيضا أن (30٪) من الرضع مع عيوب الأنبوب العصبي لديهم انكفالوسيل، (7٪) لديهم أطراف تشوه و (3٪) لديهم الشذوذ القلبية.أوضحت الدراسة وجود علاقة ذات دلالة معنوية بين نوع عيوب الأنبوب العصبي وضعف أقل للخارجية (P = 0.028- (الجدول 9)). هذه العلاقة القوية ترجع إلى أن كيس meningomylocele يحتوي على أي جزء من الحبل الشوكي أو جذر العصب الشوكي ضمن ذلك. أيضا علاقة ذات دلالة قوية بين عقاقير الأمهات خلال الأشهر الثلاثة الأولى ونوع عيوب الأنبوب العصبي قيمة P = 0.016 (الجدول 8). ، وهذا يعني أن الدواء هو عامل خطر لعيوب الأنبوب العصبيالاستنتاج: عيوب الأنبوب العصبي في السودان تحتاج إلى مزيد من الأبحاث لتقديرنسبة انتشارها ,أنماطها وعوامل الخطر التي تؤدي اليها لتشكيل قاعدة قوية لتطبيق البرامج الوقائية مثل التكميل مع حمض الفوليك.التوصية: رفع مستوى الوعي لدى الجمهور تجاه عوامل الخطر وأسباب عيوب الأنبوب العصبي واستهلاك حمض الفوليك قبل الحمل من خلال تقديم رسائل فعالة في مجالي الإعلام والتعليم والاتصال من خلال وسائل الإعلام وغيرها من القنوات. وإنشاء برامج لمنع ومكافحة هذه العيوب ضروري، وتحسين نظام غذائي للمرأة طوال سنوات الإنجاب، وضمان كمية غذائية كافية من الفيتامينات والمعادن على وجه التحديد حمض الفوليك.
