Objective: The aim of the study was to evaluate whether C-reactive protein (CRP) level on admission could aid the diagnostic accuracy of Alvarado score in acute appendicitis. Methodology: Prospective study hade been done betweenJune2014 to Septemper2015in Aleppo university hospital. We evaluated hospital records and database of 168patientswho under went appendicectomy for acute abdominal pain. Patients were categorized into three groups based on the Alvarado score. Group I: score 7–10 (n = 134), group II:score 4–6 (n = 27), group III: score less than 3 (n = 7). Negative (NPV) and positive predictive values (PPV) of , Alvarado, CRP and together was calculated.Result:168 patients underwent open appendectomy in the study period. males were 88(52.4%) and females were 80 (47.6%). Common age group was between 11-20years (43%)with mean age 17.7 years. Overall, 34of 168(20.2%) had histopathological confirmation of normal appendice .CRP levelmeanwas36.15 mg/dl in 109(64.8%) of acute appendicitis patients and 67.16 mg/dl in 25(14.8%) of complicated appendicitis patients .The predicted accuracy of Alvarado score was 85.8% in group I, 59% in group and 28.5% in group III. The PPV of high CRP and NPV of normal CRP for group I was 91%and 45%, in group II, 86.6% and 75%, in group III, 50% and 80%, respectively. Conclusion: The Alvarado score and CRP taken together improve the predictive value of diagnosing acute appendicitis.
