Sensitivity and Specificity of Cytological Techniques for Detection of Extrapulmonary-TB, Red Sea State, Sudan, 2015 – 2017

الملخص

Extrapulmonary-Tuberculosis is a major health problem in studied area,causing high morbidity and mortality, so the early diagnosis with proper microbiological and cytological diagnostic accuracy is highly and urgently needed. This cross-sectional descriptive study conducted in Port Sudan, Red Sea state, Sudan in the period from April 2015 To March 2017. This study aimed to evaluate the sensitivity, specificity and the predictive values of cytological techniques for the detection of Extrapulmonary-Tuberculosis Cases in the study area with limited Facilities . The total number of specimens were 200. Male to female ratio was 1.4 : 1 . ExtraPulmonary-Tuberculosis Specimens which include lymph nodes aspirates, Pleural fluids and Ascetic fluids they were collected under a septic conditions via Fine needle aspiration technique in sterile containers at the out-patients clinics of Sea ports corporation Hospital , Port sudan Teaching Hospital and others medical centres in port Sudan and submitted to Bacteriology and cytology laboratory of Rea sea modern medical centre. From each specimen, a sample was inoculated in Lowenstein Jensen media. Six smears were prepared by concentration method, from each specimen . Then the smears were stained by Ziehl-Neelsen stain, Harris Haematoxylin and MayGrunwald Geimsa stains. In culture method, which was consider as the gold standard method, 108 ExtraPulmonary-Tuberculosis specimens were positive and 92 Extrapulmonary-tuberculosis specimens were negative. The sensitivity and the specificity of the cytological techniques were 94.4% and 94.5% respectively. Regarding the Ziehl-Neelsen stain the sensitivity and specificity were 81.4 and 89.1 respectively. The quality of Harris Haematoxylin staining for the nucleus was 90% , for the cytoplasm 85% and for the background 80%. For the MayGrunwald Geimsa stain the quality of the nucleus staining were 80% , for the cytoplasm were 70% and for the background were 80% . According to this researche , the cytology was regarded as the best method for detection of the ExtraPulmonary-Tuberculosis cases, because it more sensitive and specific.The Harris Haematoxylin gave better results than MayGrunwald Geimsa stain. Further researches regarding molecular biological techniques should be considered in the future.
