Introduction and Aim: Cranial base plays a key role in the craniofacial growth; but its role in malocclusion still unclear. This three dimensional study aimed to compare cranial base geometry between class II division I and class III cases using three dimensional cone beam computerized tomography 3D-CBCT.Materials and Methods: the Sample consisted of (35) adult subjects (17 male, 18 female), aged (18-25 years), two groups: class II group (n=20) and class III group (n=15). Several 3D linear and angular measurements, obtained through computerized analysis of the cone-beam computed tomography images that were taken of each case, were analyzed and correlated. Comparison was conducted between the mean values recorded depending on the class and sex using statistical analysis of T Student for independent samples .Results: There were no statistically significant differences in cranial base geometry between Class II and Class III cases, but the cranial base angle (N-S-Ba) was greater in class II more than class III without significant difference. The results also showed that the most of linear measurements variables in males were significantly larger (P< 0.05) than female. No significant difference (P>0.05) was found between the mean angular measurements variables in most of the variables depending on the sex of the patient, except of variable angle (Ba-PCP-FC) was significantly larger in females than in male. Conclusion: Results indicated that there was a lack of specialty at the cranial base geometry between class II division 1 and class III of malocclusion, although there were some differences with no statistically significant e.g. cranial base angle. In general, cranial base in females was smaller and narrower than males, while it tended to be more flexure in males
