Cervical cancer is the 2nd most common form of cancer women in worldwide. The prognosis and management of this cancer require a clinical and paraclinical classification and a balance sheet extended to the neighborhood bodies including bladder. The extension of bladder cancer of the cervix is fairly common and should be recognized before the treatment decision. Cystoscopy examination is recommended by FIGO to highlight the expansion bladder. CT and pelvic ultrasound were included in the assessment of extension of cervical cancer in particular to find the bladder invasion, even if they are not part of the review recommended by FIGO. The aim of this study was to clarify the accuracy, the sensitivity and the specificity of computed tomography and transabdominale ultrasound for bladder invasion by cervical cancer in comparison with cystoscopy. This research is a retrospective study conducted among 62 patients with cancer of the cervix and who have benefited from a cystoscopy at the urology department at the University Hospital Center Mohammed VI in Marrakech, for a period of 6 years extending from the first January 2003 to December 2008. The results obtained by computed tomography (CT) and transabdominal ultrasonography were compared with those of cystoscopy. According to the FIGO classification we have 4 cases of stage I, 33 cases of stage II, 18 cases of stage III and 7 cases of stage IV. Cystoscopy showed a bladder invasion in 28 cases or 45.5%, all of these cases have also been detected by CT, which revealed bladder invasion in 5 other patients but without confirmation in cystoscopy. The renal and bladder ultrasound was performed in 30 patients and showed a bladder invasion in 12 patients or 40% of cases. NPV and sensitivity of CT to identify bladder invasion are both 100%. Those of ultrasonography were respectively 67% and 66.66%. Practice of a cystoscopy examination has no use for stages I and IV, and should not be considered when CT shows no bladder invasion by cervical cancer for all stages. It also loses its usefulness to a negative ultrasound result for stage I and to a positive outcome for stage IV. Regarding the stage II and III and when the CT is positive, it is difficult to determine given that our results are compared with cystoscopy not surgery. However, we recommend the use of new imaging techniques more reliable and less invasive to spare our patients already suffering from their cancer undergo invasive cystoscopy and not pleasant to realize.
