الملخص

Les poches de rétraction tympaniques représentent une forme particulière de l’otite moyenne chronique, réalisant une atélectasie localisée ou globale de la membrane tympanique.L’évolution imprévisible de ces poches vers l’otite cholestéatomateuse explique la gravité potentielle de cette affection.Nous rapportons une étude rétrospective à propos de 65 cas de poches de rétraction colligées dans le service d’ORL et de chirurgie cervico-faciale du CHU mohammed VI de Marrakech, sur une période de 5 ans de 2005 à 2010. Une analyse épidémiologique, clinique, paraclinique, thérapeutique et évolutive a été faite.La moyenne d’âge était de 24 ans. Neuf patients avaient une otite séromuqueuse à l’enfance, traitée par aérateur trans-tympanique. L’hypoacousie a été le symptôme révélateur dans 83.O7% des cas, associée à des otorrhées dans 66.15% des cas et à des otalgies dans 24.61% des cas.Un examen audiométrique doit toujours être réalisé afin de juger le retentissement fonctionnel. Ainsi, la majorité de nos patients présentaient une surdité de transmission de plus de 20 dB.Une antro-atticotomie a été réalisée dans 49 cas. Un cholestéatome associé a été retrouvé dans 14 cas. Tous les patients ont bénéficié d’un renforcement tympanique par cartilage, dont l’origine était conquale dans 55.38% des cas et tragale dans 44.61% des cas.Le traitement chirurgical des poches de rétraction impose le choix d’une technique permettant l’exploration adéquate de l’oreille moyenne, le rétablissement de l’effet columéllaire et la prévention de récidive.Une surveillance au long cours est nécessaire pour apprécier la stabilité des résultats de la prise en charge.

The tympanic retraction pockets are a special form of chronic otitis media, producing a localized or global atelectasis of the tympanic membrane.The unpredictable evolution of these pockets to cholesteatoma otitis explains the potential severity of such condition.We report a retrospective study of 65 cases of retraction pockets collected in the ORL department of Mohammed VI University Hospital in Marrakech, over a period of 5 years from 2005 to 2010. Epidemiological, clinical, paraclinical, therapeutic and evolutive characteristics were analyzed.The average age was 24 years. Nine patients have had secretory otitis media in childhood, treated by trans-tympanic aerator. The hypoacusia was the revealing symptom in 83.07% of the cases, associated to relapsing acute middle otities in 66.15% of the cases and otalgies in 24.61% of the cases.The audiometric examination must always be carried out in order to judge the functional results. The majority of our patients presented a transmission deafness of more than 20 dB. An atticoantrotomy was performed in 49 cases. Associated cholesteatoma was found in 14 cases. All patients underwent a tympanic reinforcement by cartilage of which the origin was from the conchal in 55.38% and the tragus in 44.61%.Surgical treatment of retraction pockets requires the selection of a suitable technique for the exploration of the middle ear, the restoration of the columellar effect and the prevention of recurrence.A long-term surveillance is necessary to assess the stability of the results of the management.
