ترجمة الملخص
Aim of study: highlighting the possibility of diagnosing PNES without video EEG. Materials and methods :the study contained 38 patients (24 females and 14 males) aging (14-58) referred to the neurology clinics for evaluation of paroxysmal events as soicated with loss of conciousness. Patients were divided equally to "possible PNES" group (clinical history and seizure semiology consistent with PNES in the absence of epileptiform discharges on interictal EEG) and "epileptic seizures" group .all patients underwent physical exam, neuroglogical exam ,ECG, EEG, brain CT, depression screening and somatization screening.Results:47.4% of patients in the possible PNES group had nonspecific changes on EEG. 31.6% of patients in the possible PNES group were taking antiepileptic drugs. 84.2% of patients in the possible PNES group had at least two psychologicaltriggers versus 57.9% in the ES group(p=0.022). the PHQ2 depression screening (with ascore equal to or more than 2) was positive in 78.9%of patients in the possible PNES group versus 36.8%in the ES group(p=0.009).The somatization screening was positive in 68.4% of patients in the possible PNES group versus 36.8% in the ES group (p=0.044). Tongue biting, urinary incontinence and self injuries were more prevalent in the ES group without reaching a significant statistical value .Conclusions :A significant proportion of PNES patients are exposed to unnecessary treatment with antiepileptic medications. Depression and somatization are more prevalent among PNES patients than epilepsy patients.
