الملخص
Prostatitis is an inflammation of the prostate gland. There are four various categories of prostatitis will be described in this review and the classification of these types depends on the symptoms and the manifestation of the condition. This study has been carried out on Toll Like Receptor related with prostatitis patients who attended to the urology out patients department (UOPD) of Al-Sader Teaching Hospital in Basrah, Al-Sader Teaching Hospital in Missan south of Iraq during the period from January 2019to October 2020.This study was conducted to detect the various TLRs from Prostatitis patients by phenotypic and molecular methods with a study of titer of prostate specific antigen and detection of TLRs by flow cytometry. From total number of (135) patients with prostatitis were taken from two provinces (Basrah and Missan) from the Basrah Teaching Hospital and Missan Teaching Hospital that included in the present study. The age of patients was ranged between 40 ->70 years and135individuals regarded as control group without any urological problems were also studies. In the clinical study found that patients presented with irritative symptoms with P <0.05. The study also found that patients presented with Obstructive symptoms with P <0.05. The study also found patients presented with Other symptoms with P<0.05. And present study found that patients with Suprapubic Tenderness with P-value <0.05. And in clinical symptoms show that the age group 40-49 years with P <0.05. Age group 50-59 years found with P <0.05. The third age group 60-69 years and the final age group >70 years with P <0.05. This study shows the effect of PSA level inpatients with prostatitis and control group, with P-value <0.05therefore the study shows a positive significant between elevated PSA levels and Prostatitis. The molecular expression study include extracting DNA from blood of Prostatitis patients, Prostitis patients and Control group by using specific primers for conventional PCR and Real Time PCR, The amplification of all extracted DNA from blood samples was preform and confirm by using electrophoresis with (100volt/30min) , The result of this estimation revealed that the amplified DNA(PCR product) was 135bp for TLR1gene (50voltage for 1hour)and the presence of TLR1 was 75% from total samples , (PCR product) was 125bp for TLR2geneon agarose gel(1%) ,(50volt/1hour) with a presence of 95% ,(PCR product) was 227bp for TLR4geneon agarose gel (1%) , (50 voltage for 1hour ) with a presence 100%, (PCR product) was 149bp for TLR7geneon agarose gel (1%) , (50voltage for 1hour) with a presence 100% ,(PCR product) was 279bp for TLR10 gene on agarose gel (1%), (50volt/1hour) with a presence 80%. The results of the present study indicate that the Toll like receptor alleles associated with risk of prostatitis. the results from sequencing of TLR4 and TLR10 were shown as following there are clear convergence between TLR4 gene and that of the Gen Bank database. Where there were single mutation appeared in forward TLR4 as A to G 317 1,2. And for reverse TLR4 showed five mutations when we compared with a Gen Bank database as T to C 8221,2.T to A 296 1,2.T to G 298 1,2.A to C 301 2.G to A 406 1,2. And there is no convergence between TLR10 genes and that of the Gen Bank data base. The importance of sequencing in this study to detect if there is any mutation in TLRs genes in patients of prostatitis, There are no studies interested with sequencing of TLRs with the relation of prostatitis so we will compare our results with studies on other diseases. In flow cytometry results the present study found that TLR2 in Prostatitis patients was 94.6%, and Prostitis (prostate cancer) was 1.16% and in the control group was 48%,and found TLR4 was 98.5% in prostatitis patients, and Prostitis was 85.1% and in the control group was 97% by total monocytes of total white blood cells.
