- Giardiasis is one of the most common parasitic gastrointestinal diseases in humans worldwide. It is caused by the parasitic protozoan Giardia duodenalis, which is endemic throughout the world with a vast range of mammalian hosts. Giardia infection is transmitted by the fecal-oral route and results from the ingestion of Giardiacysts through the consumption of contaminated food or water or through person-to-person (or animal-to-person) transmission. Traditionally, diagnosis of these parasites depends on microscopic detection of cysts or trophozoites. Recently, more specific and sensitive molecular techniques have been introduced to identify these infections. G. duodenalisis considered a complex species, with Minor morphological differences but remarkable genetic variations. It can be genetically differentiated into eight major assemblages: A to H, which differ in their host distribution. Because there is little epidemiological information about Giardia parasite and lack of genotyping studies of this parasite in Syria, this study aimed to apply molecular analysis methods as a sensitive way of accurate diagnosis and to evaluate the prevalence and diversity of Giardia assemblages, sub-assemblages causing humans infection in the city of Damascus and its suburbs. 60 samples collected from three major hospitals in Damascus city during the period of nine months (from June 2015to February 2011).Direct microscopic examination was performed. The genomic DNA was extracted then ß-giard in gene was amplified. Giardia positive samples were genotyped using PCR/RFLP assay targeting the ß-giardin and 4glutamate dehydrogenase (gdh) genes. HaeIII, BspL1and RsaIrestriction enzymes were used to differentiate between G. duodenalis assemblages /sub-assemblages.Our data showed that the percent of positive samples examined by microscope was 66.7%, whereas the positive isolates tested by PCR was 83.3%. In addition, 66% of isolates were of assemblage A; 50% belonged to sub-assemblage AII and 16% to sub-assemblage AI. Assemblage B was detected in 24% of isolates; 12% fit in sub-assemblage BIV, 4% fit in sub-assemblage BIII and 8% fit in Discordant genotype BIII/BIV. Mixed genotypes were identified in 10% of isolates; 6% fit in AII+BIII and 4% fit in AI+BIV. Significant correlation was found between Giardia AII sub-assemblage and weight loss symptom (P-value= 0.046) as well as between contact with domestic animals (cats, P-value= 0.024). Moreover, a significant correlation was found between sub-assemblage AI and livestock breeding (P-value= 0.000). This is the first molecular study performed on Giardia isolates in Syria, which indicates the diagnosis of Giardia using molecular tests is more sensitive than traditional methods and the genotyping of human G.duodenalis isolates suggests anthroponotic transmission for the route of infection in Damascus and its suburbs.
