الملخص
Nonorthopedic surgeries are associated with the risk of developing venous thromboembolism (VTE) as one of the most common preventable postoperative complications; which comprises deep venous thrombosis (DVT) and Pulmonary Embolism (PE). This study aimed to evaluate the practice pattern of venous thromboembolism after major non orthopedic surgery at Wed Medani Teaching Hospital. A mixture of an observational retrospective and operational cross sectional studies were used. At the first part of the study, 321 of patients who candidate for thromboprophylaxis (at low, moderate and high risk) were enrolled. There was no risk assessment for VTE and bleeding complications, although 87% (n=279) of patients were found to be at moderate and high risk of thrombosis; only21% (n=67) of them had been prescribed for thromboprophylaxis. The dosage regimens of the pharmacological thromboprophylaxis weren’t prescribed according to the recommended guidelines. At the second part of the study; 71of doctors working at surgery department were included to evaluate their awareness about thromboprophylaxis after nonorthopedic major surgery, forty percent of them recommended pharmacological thromboprophylaxis as a strategy to be used.; about Fifty Percent of the participants estimated that thromboprophylaxis is used only at first day postoperative. When assessing the overall awareness; 24 marks was given to knowledge parameters, the maximum mark was 16 (less than 20); there was a poor knowledge. This study conclude that, the practice pattern of thromboprophylaxis after non orthopedic major surgery at Wad Medani teaching hospital surgery was inappropriate, furthermore knowledge and awareness gaps were existing among surgical doctors regarding to VTE prophylaxis practice pattern. Finally recommended that; the selection of VTE prophylaxis should be offered by the clinical team including a clinical pharmacist to ensure that patients consistently receive adequate prophylaxis regarding to their VTE and bleeding risks.
