- Acute headache is a common complaint among patients seeking care at an emergency department. The primary role of ED physicians is to discriminate between the most common primary benign headaches and secondary potentially life-threatening headaches. There are several tools at the physician’s disposal to corroborate the diagnosis (anamnesis, examination, neuroimaging, lumbar puncture). This work which resumes the review of literature, comments retrospectively 50 observations of acute headaches gathered in the emergency department of Avicenna Military Hospital, during the period dating between January 2006 and December 2007. We tried to describe the diagnostic and therapeutic approach undertaken by the ED physicians. Our study focused on 50 patients (28 men and 22 women), their age ranging is between 17 and 78 years with a median of 49,2 years. The diagnostic criteria met those of the International Headache society. 37 patients were diagnosed with primary headaches, whom 28 of them with migraine, whereas only 13 had secondary headaches, whom 8 of them had a brain lesion. The brain scanner was performed in 60% of cases and showed abnormalities in 23% of cases (7 patients). 14% of patients needed reanimation measures; medical treatment has been undertaken at 84% of patients with using the triptans in 17% of cases, 4 patients had a surgery. 35 patients had a medical follow-up, including 20 cases of migraine, 6 tension-type headache and 9 secondary headaches. 61,5% of followed patients with primary headaches had a good evolution, 19% among them (3 patients) had wet-cupping therapy. We report two deaths which were caused by a post-traumatic subarachnoid hemorrhage and a cerebral hematoma. The overwhelming majority of headaches are primary; however it is capital to eliminate an organic or a life-threatening disease.
