الملخص
A case-control study has been carried out on autistic spectrum disorder patients who have been registered at Ausraty Center and other children who have been diagnosed by psychiatrist and pediatric doctors in Basrah, southern Iraq during the period from December 2018 to October 2019. A total of 194 children were enrolled in this study, 94 were patients with ASD and 100 were apparently healthy individuals, as control group, matched with patients' age and sex (2 -13 years). This study was conducted to detect, the plasma levels of Transforming growth factor-Beta-1 TGFβ-1, Interferon Gamma (IFN-γ) and Anti Myelin Basic Protein (AMBP) by Enzyme Linked Immuno-sorbent Assay (ELISA) technique, the frequency of alleles and genotypes of gene polymorphism of TGFβ-1 and IFN-γconventional Polymerase Chain Reaction (PCR) with special sets of primers, the percentage of CD4, CD8,and CD25 marker within 48 hours by using flow cytometry technique and lastly frequency of Human Herepes Virus -6 type A and B (HHV-6A,HHV-6B)infection by Real time polymerase chain reaction qPCR. The study revealed that autistic spectrum disorder patients have lower concentration of TGFβ-1(95.91ng/ml±6.37) than control group (117.08 ng/ml ±7.58). This difference was statistically significantly, While IFN-γ and AMBP reported a significantly higher concentration in ASD patients (24.87 pg/ml±2.003,28.14 pg/ml ±1.77) than control group (18.86 pg/ml ±1.38,16.05 pg/ml ±0.95). Regarding the results of Single Nucleotide polymorphism (SNP) a statistically significant difference was resulted in the frequency of TGF-1gene polymorphism in both position (condon 10 and codon 25); in codon 25 G/C alleles, the frequency of C allele was more in the autistic patients than control while in codon 10 The frequency of T allele significantly higher in autistic group than the control. In this study there is a moral significance repetition result of TC genotype in TGF-β1 gene polymorphisms codon 10 +860 T/Cin ASD patients compared with control samples. Interestingly, the TC, allele have been associated with higher TGF-β1plasma level so the study found a significant correlation between the plasma level of (TGF-β1) and the TC genotype of Codon 10 in ASD group which was (79.0) in compared to the control group (55.0). Another crucial SNP locus is codon 25G/C which may impact the production of TGF-β1,the study found that the CC genotype significantly higher in ASD patients than the control group and the frequency of allele C at TGFβ-1codon 25 was significantly higher in patients with ASD than in healthy control. Concerning IFN-γ+874 T/A gene polymorphism the findings showed there was a significance differences in the frequency of T allele in the autistic (23.4 %) and control group (67 %), p value < 0.05 ,and AA genotype has been related to higher plasma protein level of IFN-γ, compared with the TA genotype which relation to lower plasma protein level of IFN-γ, while the homozygous (TT) genotype has been characterized by intermediate levels. Flow cytometry based assays for cell counting showed that patients with autistic spectrum disorder had decreased counts of CD4, CD8 and CD25 than control group which scored mean (25.47 vs 28.37 , 27.42 vs 30.47 and 3.91 vs 4.91) respectively, indicated significant differences undergo P value <0.05. The distribution of HH6 A virus in case and control groups were showed no significance variation in the percentage of HH6A positive among autistic and the control groups ( P >0.05), while HHV-6 B were detected in (35.1%) of autistic patients versus (2%) of control children and statistically the difference was significant (P value < 0.038).From the results of the current study, it can be concluded that the plasma levels of TGF-β1,INF-γ, AMBP and gene polymorphsim of TGF-β1 and IFN-γ has been associated the ASD. A much larger number of patients need to be screened for HHV-6 genotype B infection and to outline the clinical determinants of this complication and assess any possible association with other biological factors, including the levels of TGF-β1,INF-γ, AMBP and counts of CD4,CD8 and CD25.The present study was clarified a positive correlation between the presence of viral infection (HHV-6B) and the plasma protein level of TGFβ-1 in ASD patients.
