Object: Comparative Retrospective study to assess the results of opens plenectomy of with laparoscopic route in advantages and disadvantages in AUH and comparing these with other studies. Goals: Comparing the two methods of splenectomy of surgical results of the -Bleeding during and after surgery.-Operationtime.-Complications.-Drainages.-Hospitalization period. Period:5 years (2006-2011).Materials and methods: Full history for all admitted patients with full medical examination and the necessary investigations .The patients then were divided into 2 groups (open surgery group and laparoscopic surgery group) and each group was studied compared to the other ..The cases which was not capable for laparoscopic operation and they are the following:-Massive splenectomy (more than 20 cm).-Spleen weight more than 1000 g.-Extreme obesity.-trauma.-Pregnancy.-prior abdominal surgery.-Portal hypertention.-Sp. Artery calsification. Results:-29cases were included in the study, 48.12% of which was in the laparoscopy. 89-Females69%.-Mean age was 21.7 years (range 14-38) with the second decade as the most common subgroup.-ITP was the most common indication 73.3% for OS and 62.3% for LS. -Mean spleen diameter was 19 for OS and 16.8 for LS.-Drainages were 33.3% for OS and 14.2% for LS.-Overall complications rate was 53.3% in OS and 28.6% for LS.-Mean Operation time was 78min in open surgery group vs 122min in laparoscopic surgery group. -Mean hospitalization period was 1.2days in group 1 vs 3.9 days in group 2.Conclusion:Laparoscopic surgery for splenectomy has similar efficacy as the open surgery with less complication and hospitalization period but longer operation period.
