Reliability of Sudanese Practical Assessment Examination Skills Soba Center Training and Examination, Soba University Hospital Khartoum State Sudan 2014

SPACES is the Sudanese practical Assessment of Clinical Examination Skills. It is the most important part of the final clinical examination of the internal medicine council of the Sudan Medical Specialization Board (SMSB). SMSB is the only institute authorized to certify postgraduate clinical degree in the country. SPACE is a new method adopted by internal medicine council over the last 4 years to replace of the long/short method, its inter-rater reliability has not been assessed. This is a retrospective analytic study of the inter-rater reliability of the Sudanese Practical Assessment Clinical Examinations (SPACES) using Cohen’s Kappa as measurement of the inter-rater agreement. The results of the exam held in December 2014 were selected after using the SPACES being used four times.70 candidates sat for this exam. Their results were obtained from archive of the internal medicine council (IMC) of the SMSB. Each candidate was assessed in seven stations (this is called a circuit) by two examiners in each station. The examiners marked the candidate independently. The exam was conducted in one day. There were four similar circuits on the exam day; each circuit assessed 17-18 candidates. The total number of the examiners was 56 (some of them have being trained by the royal collage of physician of UK and are PACES examiners), and the total number of the stations was 28. Each circuit runs two cycles (8-9 candidates each). The cases in the four circuits were of similar difficulty. A total of 980 mark sheets (results) have being analyzed. The average Kappa of the whole exam was 0.161 which mean there is slight agreement. The average Kappa of circuit one, two and four showed slight agreement (Kappa 0.01, 0.20, 0.21 respectively) while circuit three showed fair agreement (0.4) according to Cohen’s Kappa.Kappa of four stations (14.8%) out of the 28 stations were less than zero, which means agreement of less than by chance. Kappa of twelve stations (44.4%) was 0.01 to 0.20, means slight agreement. Kappa of eight stations (29.9%) was 0.21 to 0.4, means fair agreement. Kappa of three stations (11.1%) was 0.41 to 0.60, moderate agreement. Probably the execution method used in this exam which include, large number of the candidates the large number of probably untrained examiners and the large number of the patient needed for such exam in one day all could contribute to this low inter-rater reliability.In conclusion: the reliability of SPACES in December 2014 was slight according to all the models used to interpret Kappa degree. However there is a good opportunity to improve the reliability of the SPACES due the presence of examiners in the SMSB who have being trained by the royal college of physician and are now examiner in PACES. The method of execution may need some changes.
