الملخص

Notre étude est une enquête qui avait pour but de faire une évaluation économique du coût des implants orthopédiques dans le traitement des fractures. 84 patients ont fait parti de cette enquête qui s’est étalé de novembre 2004 à septembre 2005. L’âge moyen était de 40ans. 59 patients (70%) étaient de bas niveau socio-économique. 65 patients (78%) avaient une fracture du membre inférieur. Nous avons calculé le coût moyen des implants achetés par les malades (Cm1) et la durée moyenne de séjour globale. Nous avons également pris à part les malades ayant subit une ostéosynthèse interne. Nous avons calculé pour ces malades le coût moyen du matériel d’ostéosynthèse (Cm2), la durée moyenne de séjour préopératoire et la durée moyenne de séjour globale. Nous avons calculé le coût moyen du matériel d’ostéosynthèse (Cm3) sur la base des données d’un marché publique réalisé par le CHU Mohammed VI à Marrakech pour l’acquisition d’une quantité limitée de matériel d’ostéosynthèse. Nous avons enfin calculé la durée moyenne d’hospitalisation optimale pour les patients ayant subit une ostéosynthèse. Le coût Cm1 était de 2384 DH. Le coût Cm2 était de 1826DH. Le coût Cm3 de était de 712DH. La durée moyenne du séjour globale était de 18 jours. La durée moyenne de séjour pour les patients ayant subit une ostéosynthèse interne était de 17 jours. La durée moyenne de séjour préopératoire pour ces patients était de 9,6 jours. Leur durée moyenne de séjour optimal était de 3,4 jours. Au vu de ces résultats, on voit que les structures hospitalières seront largement gagnantes en mettant à disposition des chirurgiens du matériel d’ostéosynthèse. Elles gagneront en économisant sur les journées d’hospitalisation inutiles.

Our study is an investigation which purpose was to make an economic evaluation of the cost of the orthopaedics implants for the treatment of the fractures. 84 patients made party of this investigation which was spread out from November 2004 to September 2005. The median age was 40ans. 59 patients (70%) were of low socio-economic level. 65 patients (78%) had a fracture of the lower limb. We calculated the average cost of the implants bought by the patients (Cm1) and the average total stay duration. We also took separately the patients having undergoes an internal osteosynthesis. We calculated for these patients the average cost of the material of osteosynthesis (Cm2), the preoperative duration of stay and the total duration of stay. We calculated the average cost of the osteosynthesis material (Cm3) on the basis of a market public data carried out by the CHU for the acquisition of a limited quantity of osteosynthesis material. We finally calculated the average of optimal hospitalization duration for the patients having undergoes an osteosynthesis. The cost Cm1 was of 2384 DH. The cost Cm2 was of 1826DH. The cost of Cm3 was of 702DH. The total stay duration average was 18 days. The average stay duration for the patients having undergoes an internal osteosynthesis was 17 days. The preoperative stay duration for these patients was 9,6 days. Their optimal stay duration was 3,4 days. Within sight of these results, we see that the hospital structures will largely be gaining while placing osteosynthesis material at the disposal of the surgeons. They will gain while saving over the useless days of hospitalization.
