Estimation of Serum Troponin I level among Diabetic Hypertensive Patients Attending to Aldaraga Health Center, Wad Medani, Gezira State, Sudan (2021)

الملخص

Cardiovascular disease is the most common cause of death among people with diabetes and hypertension, Myocardial infarction and other cardiovascular diseases may occur in this group of patient due to uncontrolled or poorly controlled diabetes and hypertension. Myocardial Injury results to a disruption of the intracellular contractile proteins leading to an increase in the cytoplasmic pool content of cardiac troponin I, This was quantitative case control study conducted in Aldaraga health center, Wad Madani City, Gezira State, Sudan from February to April 2020 aimed to measure serum cardiac troponin I as predictive marker in diabetic hypertensive patients, sixty blood samples (20 control and40 case of both gender 20 male and 20 female aged between 40 to 70 years with mean of age 55 years), samples were collected in two containers, lithium heparin for serum troponin I and EDTA for HbA1c then processed by Bio Time FIA Analyzer. Data were analyzed using statistical package for the social sciences (SPSS) version 20.the result showed significant increase in HbA1c and serum troponin I in study population compared to control group (p=0.00) while there were no difference when compared to gender (p=0.221 ,0.528) respectively, The results showed the serum level of cardiac troponin I was significantly increased in patients with diabetic hypertension disease aged between 61 to 70 (p=0.00) .compression between serum cardiac troponin I among controlled and uncontrolled diabetic hypertensive patient showed significant increase in troponin I with (p=0.014) . There was positive correlation between serum troponin I level with age, HbA1c and duration with correlation coefficient (0.69, 0.59, 0.40) respectively. The study concluded that Diabetes and hypertension had effect on cardiac troponin I especially on uncontrolled diabetic hypertensive patients. The study recommended measurement of serum cardiac troponin I level must be used as routine test for all patients with diabetic hypertension disease to early detection of cardiac disorder
