الملخص

Diarrhoeal diseases, dysentery and enteritis in children underfive years of age are leading causes of illness, dehydration, malnutritionand important cause of death in infants. Diarrhoeal disease in children isself limiting and can be treated withrehydration and other supportivetherapy without antibiotics. The antibiotics are used onlyfor treatment ofdysentery, cholera and persistant diarrhoea. In the present study, 275 stool specimens were collected, 180(65.5%) from Muzzamil Abayazid Hospital in Mayo District, KhartoumState and 95(34.5%) from Ibrahim Mallik Teaching Hospital in Elsahafa,Khartoum State during the period from November 2001 to October 2002from children under five years of age with diarrhoea. After culture inselective and differential media 283 clinical isolates were obtained. The commonest etiological organism was E. coli107(37.9%)followed by Proteus 67(23.7%), Salmonella12(4.2%), Shigella11(3.9%), Klebsiella40(8.8%), Pseudomonas 25(14%), and Citrobacter10(3.5%).The antibiotic susceptibility of all isolates showed highresistance to commonly used antibiotics, 84 of the clinical isolates ofE. coliwere resistant to ampicilin, 80 totrimethoprim 73 to tetracycline,70 to sulphamethoxazol, 40 to chloramphenicol, 39 to amoxycillin,F28 to nalidixic acid 21 to gentamicin and 10 were resistant tociprofloxacin. Proteus, 49 isolates were resistant to ampicillin andtetracycline 50 were resistant to trimethoprim, 43 were resistant tosulphamethoxazole, 38 were resistant to amoxycillin, 34% were resistantto cholramphenicol, 27 were resistant to nalidixic acid, 10 were resistantto gentamicin and 4 were resistant to ciprofloxacin.Salmonellasp all isolates were resistant to ampicillin, 8 wereresistant to tetracycline and sulphamethoxazole, 7 were resistant toamoxycillin and nalidixic acid, 6 were resistant to trimethoprim andchloramphenicol and 4 were resistant to gentamicin. No isolate ofSalmonella was resistant to ciprofloxacin.Shigellawere 11 clinical isolates, 9 of them were resistant toampicillain, 7 were resistant to tetracycline, 6 were resistant totrimethoprim and sulphamethoxazole, 3 were resistant to nalidixic acidand gentamicin, no strain was resistant to ciprofloxacin.Klebsiella, 31 isolates were resistant to nalidixic acid, 22 wereresistant to trimethoprium, 21 were resistant to amoxycillin, 16 wereresistant to tetracycline, 14 were resistant to chloramphenicol, 13 wereresistant to ampicillin, 12were resistant to gentamicin, and 10 wereresistant to sulphamethoxazole.GAll isolates of Pseudomonas showed high resistance to allantibiotics except gentamicin and ciprofloxacin to which moderateresistance was shown. Also in this study, the specimen content, age, sex and watersource influenced the incidence. Nine specimens contained blood cellson microscopic examination. It was found also the most prevalentincidence of diarrhoeal infection exists in the age between seven monthto one year (97 cases). There was no significant difference in theincidence of diarrhoeal disease between the two sexes in this study, males were 131 (47.6%), females were 144 (52.4%). The water source isthe most important predisposing factor. The majority of inhabitantslacks a clean water supply (93%). Water for drinking and domestic use isbrought by traditional methods (a cart driven barrel) and stored in pots(zeer).
