الملخص
Pseudomonas aeruginosa is a formidable pathogen that causes infections with high mortality rates because of its ability to form biofilms and rapidly acquire resistance to many first-line antibiotics. In this study, 120 isolates were collected by transport swabs from Hilla hospitals (Al-Hilla Teaching Hospital and Al-Imam Al-Sadiq Hospital) of both genders with different ages during the period 15 September to 15 November 2020. A hundred (83.3%) of specimens were observed to have positive bacterial growth, while 20 (16.6%) of specimens were observed to have no bacterial growth. Out of 100 isolates with positive bacterial growth, 80 (80%) were P. aeruginosa isolates, while 20 (20%) isolates belonged to other bacteria. P. aeruginosa isolates were collected from different sources, including burn 45 (56.25%), wound 15 (18.75%), ear 3 (3.75%), blood 2 (2.5%), urine 4 (5%), and sputum 11 (13.75%). These isolates were initially identified by culturing on MacConkey agar, blood agar, chromogenic agar, and cetrimide agar. They were then diagnosed by performing some morphological and biochemical tests. Identification of these isolates was confirmed by the VITEK 2 Compact system. Antibiotic susceptibility testing was performed on all 80 isolates using nine antibiotics via the Kirby-Bauer method. The results showed the highest antibiotic resistance among bacterial isolates to ciprofloxacin (71%), levofloxacin (75%), aztreonam (87.5%), gentamycin (77.5%), amikacin (73.5%), tobramycin (82.5%), ceftazidime (68.75%), while the lowest resistance percentage was to imipenem (18.75%) and 0% resistance to meropenem. Among the 80 P. aeruginosa isolates, 38 (47.5%) were multidrug-resistant (MDR), of which 10 isolates were MDR and simultaneously 100% resistant to both ciprofloxacin and levofloxacin. DNA extraction from these 10 MDR P. aeruginosa isolates was conducted using a Quick Bacterial DNA Mini Kit. Identification of these MDR isolates was confirmed by polymerase chain reaction (PCR) using the universal 16S rRNA gene. Hemolysis tests on blood agar showed that 8/10 (80%) P. aeruginosa isolates produced β-hemolysis, 20% produced gamma (γ) hemolysis, and 0% produced alpha (α) hemolysis. Biofilm formation was investigated according to the tissue culture plate method (TCP), showing that 100% of isolates were positive for alginate production: 60%, 30%, and 10% of isolates represented strong, moderate, and weak biofilm grades, respectively. PCR screening for genes responsible for fluoroquinolone resistance in P. aeruginosa, including gyrA, gyrB (subunits of topoisomerase II), parC, parE (subunits of topoisomerase IV), and mexA, mexB efflux pump genes, revealed their presence in MDR P. aeruginosa isolates at 100%, 100%, 20%, 70%, 100%, and 100%, respectively. DNA sequence alignment showed 99% identity with the reference strain MT64631.1. This study revealed that all isolates carry one or more genes responsible for quinolone resistance at different rates (gyrA, gyrB, parC, parE, mexA, mexE).
