الملخص
Infertility is the inability of couples to obtain pregnancy after one year of frequent unprotectedintercourse. It is associated with important psychological economic, demographic and medicalimplications. This is a descriptive prospective study carried out during the period 1st of April to 31st of October 2002in the Police University Hospital to estimate the prevalence of primary and secondary infertility and todetermine the causes and associated factors. One hundred and twenty-four Sudanese infertile couples were interviewed. The commonest age of thefemales studied group was 31-35 years (39.5%) of which half of them reached their secondary schoollevel (50%). Nulliparous women predominated (72.5%). Almost one third of the study group hadvaginal delivery in their last delivery. Most of the husbands were in the age group 41-50 years (48.8%)of which (46.6%) were highly educated. The prevalence of infertility among the studied couples was found to be 10.4% of which primaryinfertility constituted 6.0%, while secondary infertility constituted 4.4%. The aetiological factorsincluded: anovulation (25.8%), tubal factor (18.6%), male factor (30.6%) and 25%ivwere unexplained. The study showed that 30.4% of the cases of tubal factor had symptoms suggestiveof PID and another (30.4%) preceded by a previous pelvic surgery. In the secondary infertile group, caesarean section and abortion in the last delivery carried no more riskthan vaginal delivery in relation to tubal factor infertility (P> 0.05). Those having abortion in their lastpregnancy whether evacuated or not, had no significant difference in relation to tubal factor infertility(P>0.05).Male factor and unexplained infertility showed a more prevalence in the primary infertile group. The study showed (15.8%) of the cases of male factor infertility in which the underlying cause was avaricocele, (13.2%) caused by sexual dysfunction and (2.8%) caused by undescended testicle. As infertility is an important health problem, the study recommended more concern by the healthauthorities. This involves establishing specialized infertility centres with ART management facilities inaddition to preventive measures to infertility caused by tubal factor such as treatment and follow up ofpatients with PID.
