Thoracic inlet X-ray in perioperative evaluation of endotracheal intubation difficulty and complications in patients with goiter

الملخص

In this study thoracic inlet X- ray, AP and lateralviews was assessed in 100 patients with clinically confirmedgoiter who underwent thyroidectomy to determine the valueof this investigation in diagnosing upper airway obstruction,predicition of difficulty during anaesthetic induction andanticipation of postoperative necessity for ventilationsupport in these patients. It was combined with otherpreanasthetic and anaesthetic clinical tests such asmalampatti and laryngoscopic grading for assessing theairway and laryngeal intubation as wellThis study showed that, conventional radiology is theaccepted method for detecting upper airway abnormalitiesespecially in patients with huge goiter with retrosternalextension.Thoracic inlet x-ray did not predict difficultendotracheal intubation in patients with thyroidenlargement, there were 5 patients who had difficultendotracheal intubation (DEI). It can be of help in predictionof adequacy of manual ventilation, 9 patients hadinadequate ventilation. It anticipated postoperativeventilatry complications, 6 patients needed postoperativesupport, 4 of them were reintubated while two hadtracheostomy. Conventional radiology still has a role inmanaging patients with goiter as far as it is simple, cheapand available.Combination of clinical examination and radiologicalinvestigations are important in assessing patients withgoiter when they undergo thyroidectomy and might give anidea about difficulties during operation.
