HLA-DR,DQ genotyping and IL-4, IL-18 gene polymorphism in arthritis patients

الملخص

The current study was conducted to investigate the role of some aspects of immunogenetics that related with arthritis disease. The study includes 100 blood samples were collected from people who attend to Marjan hospital, with 25 samples from apparently healthy people during the period of October 2015 to March 2016. Immunological study involved estimate the levels of IL-18, IL-4 in certain arthritis patients using ELISA technique, the estimation of IL-18 revealed significantly increased in all patients by contrast with control group with mean value 133.93 pg/ml in rheumatoid arthritis (RA) patients and 103.3 pg/ml for osteoarthritis (OA) patients (P < 0.01). Estimation of IL-4 and contrary results showed increasing IL-4 levels in both types of arthritis with mean value (45.06 pg/ml, 33 pg/ml) with RA and OA respectively compared with control group (22.48 pg/ml) with P ≤ 0.01. The genetic study involved screening of IL-18 (-137 G/C) and IL-4 (-590 C/T) related with arthritis patients for 75 samples including patients and control using PCR-SSP technique. The SNP of IL-18 (-137 G/C) showed: GC heterozygous with (39), GG homozygous with (21) and CC homozygous with (15). The allelic and genotypes association appeared no significant differences between patients and control groups. Another SNP of IL-4 (-590 C/T) showed: CT heterozygous with (50), CC homozygous with (9) and TT homozygous with (16). The allelic and genotypes association showed no significant differences between patients and control groups. The possibility of detection of histocompatibility antigens HLA (DRB1 & DQB1) alleles related genetically with disease by using (PCR-SSP) was performed and showed that no significant differences between RA patients with DRB1 (0301 & 0701) and DQB1 (0201) alleles compared with control, while in osteoarthritis patients showed significant differences between patients with DQB1 (0201) allele compared with control, but no significant differences with DRB1 alleles.

أجريت الدراسة الحالية للتحقيق في دور بعض الجوانب المناعية والوراثية التي تتعلق بالتهاب المفاصل. وتتمثل الدراسة ٧٥ عينة تم جمعها من الناس الذين راجعوا مستشفى مرجان، مع ٢٥ عينة من الذين يبدو عليهم أصحاء وذلك من تشرين الاول ٢٠٠١ إلى آذار ٢٠٠٢. تضمنت الدراسة المناعية تقدير مستويات الانترلوكين ٨٩ والانترلوكين ٤ في مرضى التهاب المفاصل باستخدام تقنية الاليزا. لوحظ زيادة مستوى الانترلوكين ٨٩ في جميع المرضى على العكس من ذلك مع مجموعة السيطرة بمتوسط pg/ml (133.93 في مرضى التهاب المفاصل الروماتودي) (130.3 لمرضى هشاشه العظام) مع مستوى احتمالية P≤0.01P≤0.01. أظهرت النتائج الخاصة بتقدير الانترلوكين ٤ زيادة في مستوى الانترلوكين ٤ في كلا النوعين لالتهاب المفاصل ومتوسط على النحو المبين: pg/ml ، ٤٥٠.٦ ، ٣٥٠.٦ مع التهاب المفاصل الروماتودي وهشاشه العظام على التوالي مقارنة مع مجموعة السيطرة (٧٢.٤/pg/ml). بمستوى احتمالية P≤0.01P≤0.01. تضمنت الدراسة الوراثية G/C (137 - 139 G/C) & IL-18 (590 - 571 C/T) ذات الصلة مع مرضى التهاب المفاصل ٧٥ عينة متضمنة عينات مرضى وعينات سيطرة باستخدام تقنية (PCR-SSP). أظهرت نتائج التنوع النووي النيكليوتيدي المفرد من IL−18IL−18 (137 - 139 G/C): IL−18IL−18 من متغايرة الاليل GCGC. من متماثلة الاليل CCCC كما أظهرت الأليلات والوراثات عدم وجود فروق ذات دلالة إحصائية بين المرضى ومجموعة السيطرة. أظهرت نتائج التنوع النووي النيكليوتيدي المفرد من IL−18IL−18 (590 - 571 C/T) لوحظ له: «من متغايرة الاليل CTCT من متماثلة الاليل CCCC و«من متماثلة الاليل TTTT» كما أظهر عدم وجود فروق ذات دلالة إحصائية بين المرضى ومجموعات السيطرة في حالة الأليلات والمورثات».
