Antimicrobial resistance is one of the most serious problems that faced human health in 21st century. Plasmids mediated colistin resistance (mcr-1 gene) is one of the mechanisms by which bacteria resist multiple antimicrobial agents. Bacteria that possess this gene are difficult to treat.Aim: To detect the presence of mcr-1 gene in gram negative bacteria clinical isolates. Methods: This is a cross sectional laboratory based study conducted in Khartoum state, a total of 105 clinical isolates of gram negative bacteria were obtained from different hospitals isolated from different clinical specimens including urine, wound swabs, sputum, blood, and others. These strains were re-identified using conventional biochemical tests. Antimicrobial sensitivity testing was done using desk diffusion method, followed by DNA extraction using boiling method then conventional PCR was performed to detect mcr-1 gene. Obtained data were analysed using SPSS 20.Results: A total of 105 gram negative bacteria were collected from 105 clinical specimens (one organism from each specimen) urine represented about 50% (52/105), followed by skin and wound (19%). E. coli was the most frequent isolate 36.2 % followed by Klebsiella spp 32,3%.Antibiotic sensitivity showed that 7.6% of all isolated bacteria were resistant to Colistin and 92.4% were sensitive. mcr-1 gene was detected in 4 isolates (3.8%). Statically there was no significant association between the presence of mcr-1 gene and the site of infection (p value = 0.62), type of isolated bacteria (p value =0.43) and the antimicrobial profile. Conclusion: The study found that the prevalence of mcr-1 gene is low in the isolated bacteria.
