Les Thrombopénies en réanimation au CHU Mohammed VI

La thrombopénie est la présence d’un nombre de plaquettes inférieur à un seuil, déterminé dans notre étude à 100.000mm3. C’est une anomalie fréquente qui peut être constatée dans de nombreuses situations pathologiques, chez les patients en réanimation. Ce travail a pour objectif de préciser l’incidence de la thrombopénie en réanimation, les facteurs associés à sa survenue et le devenir des patients thrombopéniques. Il s’agit d’une étude rétrospective, réalisée dans le service de réanimation médico-chirurgicale du CHU Mohammed VI de MARRAKECH sur une période de 36 mois (Janvier 2009 à Décembre 2011). Sur la période d’étude, 930 patients ont été admis au service de réanimation, nous avons recensé 110 épisodes de thrombopénie, soit une incidence de 12 % avec un taux de mortalité qui dépassait 50%. Le sepsis, la présence d’un état de choc, les traumatismes, la transfusion massive, la présence d’un cathéter central et l’admission post opératoire des chirurgies lourdes sont présents d’une façon significativement élevée dans le « groupe thrombopénique » par rapport au « groupe contrôle », et donc considérés comme facteurs associés au développement de la thrombopénie dans la population étudiée. D’après notre étude, on a constaté que la l’insuffisance rénale est présente de façon significativement plus marquée chez les patients thrombopéniques par rapport aux patients non thrombopéniques, avec un p<0.001, ce qui suggère la présence d’une relation « cause/effet » entre le développement de la thrombopénie en réanimation et l’insuffisance rénale. Dans le sens d’authentifier cette relation, nous recommandons de réaliser d’autres études prospectives à grandes échelles. La thrombopénie est une anomalie fréquente en milieu de réanimation, elle survient dans de nombreuses situations pathologiques et constitue un facteur de morbidité et de surmortalité.

Thrombocytopenia is the presence of a platelet count below a threshold, determined in our study 100.000mm3. This is a common abnormality that can be found in many pathological situations, in patients in intensive care. This work aims to identify the incidence of thrombocytopenia in intensive care, the factors associated with its occurrence and fate of thrombocytopenic patients. This is a retrospective study in the intensive care medical-surgical CHU Mohammed VI in Marrakech on a 36 month period (January 2009-December 2011). In the study period, 930 patients were admitted to the ICU, we identified 110 episodes of thrombocytopenia, an incidence of 12% with a mortality rate that exceeded 50%. Sepsis, the presence of shock, trauma, massive transfusion, the presence of a central line and the admission of post-operative surgeries are now heavy in a way significantly higher in the "thrombocytopenic group" by compared to the "control group" and therefore considered as factors associated with development of thrombocytopenia in the study population. In our study, kidney failure is present in a significantly greater thrombocytopenic patients compared to patients without thrombocytopenia, with a p <0.001, suggesting the presence of a relationship “cause / effect" between the development of thrombocytopenia in intensive care and acute renal. In the direction to authenticate this relationship, we recommend further studies looking at large scales. Thrombocytopenia is a common abnormality in intensive care unit, it occurs in many pathological situations and is a factor of morbidity and mortality.
