الملخص

The study included 300 samples collected from clinical and hospital environment sources. The clinical samples included patients suffering from burns, wounds, and otitis media, who were attending Hilla Teaching Hospital; whereas hospital environment samples included catheters, beds, bathrooms, fomites, walls of wards, and disinfectants at the same hospital. This study was carried out during the period November 2011 to March 2012. The age of the patients under study ranged from 2–65 years. All samples were cultured on MacConkey agar, Nutrient agar, and selective medium (cetrimide agar), then incubated at 37ºC for 24–48 hrs. Out of the 300 clinical and hospital environment samples, only 43 (14.3%) isolates belonged to P. aeruginosa, identified by routine diagnostic tests including cellular, cultural, and biochemical characteristics. The distribution of these isolates is as follows: 21 (7%) from burns, 7 (2.3%) wounds, 12 (4%) ear swabs, 1 (0.3%) catheters, 1 (0.3%) fomites, and 1 (0.3%) disinfectants samples. The results showed that burns were more susceptible to contamination with this organism. The isolates were tested for detection of their susceptibility to seven heavy metals (HM) including Copper Sulfate, Silver Sulfate, Mercury chloride, Lead nitrate, Zinc sulfate, Cadmium sulfate, and Nickel sulfate. The minimum inhibitory concentration (MIC) of the seven HMs was detected by agar dilution method and pouring method. Summary: All isolates showed different degrees of resistance to the studied HMs. Results showed a high degree of resistance (87–100%) of the isolates for all HMs in low concentrations, but they were sensitive to all HMs in high concentrations. Results of MIC in the agar dilution method for four HMs (silver, zinc, cadmium, and nickel) showed that 92% of isolates were resistant to silver at low concentrations, 89% to zinc, 86% to cadmium, and 100% of isolates were resistant to nickel at a concentration of 0.0001 M, while all isolates were sensitive to silver, zinc, cadmium, and nickel at a concentration of 0.1 M. Also, MIC for three HMs (copper, mercury, and lead) in the pouring method was detected. The results showed different degrees of resistance of this organism towards the studied heavy metals. The MIC values of Cu²⁺, Hg²⁺, and Pb²⁺ were 400–3200 μg/ml, 2.7–86.4 μg/ml, and 800–3200 μg/ml, respectively. The plasmid content was investigated for all 37 isolates of P. aeruginosa (34 from clinical sources and 3 from hospital environment sources). Results revealed that most isolates (32/37) harbored a large (mega) plasmid with a huge molecular weight that cannot be detected using the 3000 bp size marker (ladder) used in the present study. The biofilm formation by P. aeruginosa isolates was investigated. The results showed that 20/43 (47%) of isolates produced biofilm. Also, the results showed that all hospital environment isolates were biofilm producers (3 isolates), whereas only 17/40 (42.5%) of clinical isolates were biofilm producers. The relationship between biofilm production and heavy metal resistance (HMR) was studied. It was found that the HMR of P. aeruginosa isolates is not correlated with the production of biofilm. From these results, this study concludes that the increase in HM resistance was correlated with biofilm production for some (but not all) HMs used. Bacterial curing was performed for one isolate P. aeruginosa (P.3). The results showed that viable strains still resist all studied HMs.
