الملخص
Menorrhagia is an excessive menstrual blood loss that interferes with the woman’s physical, emotional, social, and material quality of life. Congenital bleeding disorders increase the frequency and severity of bleeding in menorrhagia. Human platelet antigen genes (HPA)are important to control platelet aggregation; on the other hand plasminogen activator inhibitor type-1 (PAI-1)genes is a main regulation of the endogenous fibrinolytic system. The purpose of this study was association between (PLA-1 4G /5G and HPA 1a/1b) genes polymorphisms with menorrhagia. Case control study was carried out during 2016 –2021 in Wad Medani, Gezira State, Sudan. 180 blood samples were collected from 90 females with menorrhagia (90 samples during the menstruation and 90 sample post the menstruation) for Prothrombin time(PT), Activated partial thrombin time (APTT), Platelets count, Bleeding time and platelet function. From 90 cases prepared DNA extraction in order to detection the polymorphisms of HPA 1a/1b and PLA-1 4G/5G. Otherwise 90 samples was collected from normal females as control for genetic analysis. The data were analyzed using SPSS software (V 16.0). There were highly significant differences in the means of PT, APTT and bleeding time between during and post menstruation (P value 0.000 ) each one. On the other side there was no significant difference in the means of platelets count between during and post menstruation; but there are strongly relation between platelets dysfunction within during and post menstruation (P value 0.000). According to genetic analysis; In PAI-1 genotypes a recessive gene with polymorphism (4G/4G) is higher in cases than controls group with P.value (0.047). HPA genotypes are dominant genes with polymorphisms (1b/1b) + (1a/1b) with higher frequency in cases than controls group with P.value (0.000) and more odd ratio (10.23). This study concluded that The HPA-1b/1b genotype has more affected in assessment of menorrhagia than PAI- 1(4G/4G).
