- Pantoea spp. one type of opportunistic pathogens opportunistic pathogens and associated with spreading of nosocomial infections worldwide .In this study,a total of 623 samples were collected and distributed as 483(77.52%) clinical samples of different clinical cases and 140(22.47%) environmental samples from hospitals in Al-Diwaniyah city including Al-Diwaniyah Teaching Hospital, Feminine and Children Teaching Hospital,Specialist Burns Center, Thoracic Counseling and Public Health Laboratory during the period from October 2017 to June 2018.The clinical samples were collected from patients who were outpatients and inpatients in the previously mentioned hospitals and with different ages for both sexes , then the clinical samples were divided according to their sources as following: 82 urine samples, 23 throat swabs, 49 ear swabs, 83 intravenous arterial puncture samples of dialysis patients, 27 stool samples,31 pus swabs of inflamed surgeries, 44 wounds swabs, 70 burns swabs and74 sputum samples. Environmental samples were included 30 ground swabs, 40 medical instruments swabs, 20 medical devices swabs,30 walls swabs, 20 bed swabs. The primary results of culture and biochemical tests revealed that 24 isolates were Pantoea species. The diagnosis was confirmed by using API 20Esystem and polymerase chain reaction technique. The current study recorded the occurrence of two pathogenic species of Pantoe agenus in hospitals of Al-Diwaniyah city, the first species was Pantoea agglomerans (2.27%) which showed higher percentage than second species Pantoea calida. This study found that the incidence of Pantoea species infection was increased in the age group (41-50)years (36.84%) and the rate of males was higher (57.89%) than females(42.10%).The antibiotics susceptibility test was performed to 24 isolates of Pantoea species in this study against 21 antibiotics which belong to 12classby using diffusion method. The Pantoea isolates showed high resistance rate to piper acillin (75%) ,ticarcillin-calviolanac acid (87.5%), amoxicillin-clavulanic acid (100%). The isolates also showed high resistance to third generation cephalosporins drugs including ceftazidim(100%),ceftriaxone (87.5%) and aztreonam (91.6%). With regard to car bapenim, meropenim has been shown to be effective toward allPantoeaisolates (100%sensitive). The isolates had low resistance to amikacin (8.3%).The results of the present study also revealed that Pantoea isolates were resistant to different antibiotics belong to different classes making them multidrug resistants. These isolates recorded different types of multi-resistance type XDR (Extensive Drug Resistance) after resistance to all types of antibiotics, except one class or two (54.1%).The bacteria showed multidrug resistant (MDR) resistance after resistance to three or more of antibiotics (29.1%). In addition, these bacteria recorded a resistance for all antibiotics ratio resistance (PDR)(Pandrug resistance)(16.6%).All isolates of Pantoea were tested for the incidence of virulence genes(rcsB, hpaA, acrAB, avrxacE2, avrxacE1, hrpG, hrc, rcsA) by using polymerase chain reaction (PCR) technique (Monoplex). The hrp G gene was showed the highest incidence rate among isolates whereas19 isolates (79.16%)harbored it, followed by hrc gene existed in17 isolates (70.83%),rcsA gene existed in 13 isolates (68.42%)andavrxacE2gene existed in13 isolates (54.16%)while the virulence genes (avrxacE1, rcsB, hpaA, acrAB)were not existed among Pantoea isolates. The current study also found that the bacteria Pantoea opportunistic pathogen cause to human infections , despite the scarcity of presence in the hospitals of Al-Diwaniyah city of, such as urinary tract infections and digestive tract infections and respiratory infections as this bacteria resistant to common antibiotics use
