Study on Prothrombin Time International Normalize Ratio and Activated Partial Thromboplastin Time among Patients with Hypothyroidism National Cancer Institute University of Gezira Sudan 2020

الملخص

Hypothyroid modify physiological process of primary and secondary haemostasis and may lead to a higher bleeding risk. This is an analytical case control study aimed to study prothrombin time, international normalize ratio and activated partial throm boplastin time among patients with hypothyroidism and to compare results between healthy individual. The study conducted in the National Cancer Institute, during the period from January to October 2020. The study population were 50 patients with hypothyroidism, and 50 normal health individuals. A questionnaire was used to obtain patients’ information as age, gender, family history. The participants were orally interviewed after an informed verbal consent was taken. One point eight ml of venous blood was collected from all the participants in trisodium citrate container then centrifuged for 15 minute to obtain platelet poor plasma. Control M4 was used for measuring activated partial time and prothrombin time, data analysis by using SPSS program. The majority of participant in study group were female (82%) while male were (18%) and according to participant age 31-40 is most common age represent (36%), 30% of the participant have family history disease .The result showed activated partial thromboplastin time value significantly increased (P 0.000) in patients with hypothyroidism, while there no significant variation observed in the prothrombin time (P.value 0.849), and international normalize ratio (P. value 0.576) among the study population. According to severity of disease there were 43 (86%) of patients with mild hypothyroidism (Thyroid stimulating hormone ≤ 40mU/L), and 7 (14%) patients displaying severe hypothyroidism (Thyroid stimulating hormone > 40 m( . Activated partial thromboplastin time results were showed there were significant negative correlation with T4 and T3 level. While prothrombin time and international normalize ratio had no significant negative correlation, activated partial thromboplastin time results were showed there were significant positive correlation with Thyroid stimulating hormone level, while prothrombin time and international normalize ratio had no significant correlation. The study concluded that activated partial time was significantly increased to patient with hypothyroidism and recommended to evaluate routinely in patients with thyroid dysfunction.

خمول نشاط الغدة الدرقية يؤثر على عملية التخثر لوقف نزف الدم في عمليته الاساسية والثانوية ويمكن ان يؤدي الى حدوث نزيف .هذه دراسة تحليلة وصفية تهدف لدراسة وقت البرثرومبين والنسبة المعيارية الدولية لوقت البرثرومبين ووقت النشيط الجزئي لتجلط الدم في المرضى الذين يعانون من خمول نشاط الغدة الدرقية. أجريت الدراسة في مركز الطب النووي في ولاية الجزيرة خلال الفترة من يناير إلى اكتوبر 2020. يشمل مجتمع الدراسة 50مريضا يعانون من خمول نشاط الغدة الدرقية و 50شخصا عاديا وهم عبارة عن المجموعة الضابطة. تم استخدام الاستبيان للحصول على المعلومات من المرضى كالعمر والجنس وتاريخ المرض في الأسرة وأجريت المقابلات مع المشاركين من المرضى شفويا بعد أخذ موافقتهم كتابة ولفظا. ، وقد تم جمع 8.1مل من الدم الوريدي من جميع المرضى ومن المجموعة الضابطةفى مضاد تجلط سترات الصوديوم الثلاثي وتم تدوير جميع العينات في جهاز طرد مركزي لمدة 15دقيقة للحصول علي بلازما خالية من الصفائح الدموية لإجراء إختبارات التجلط وقد تم استخدام جهاز لقياس تجلط الدم في اختبار وقت التنشيط الجزئي لتجلط الدم ووقت البروثرومبين. وتم تحليل النتائج المتحصل عليها بواسطة برنامج الحزم الإحصائية. في حالة قصور نشاط الغدة الدرقية كانت الغالبية من الإناث، وتراوحت أعمارهن ما بين 31-40عاما ويمثلون) 36%( من مجموعة الدراسة ، وكان 30٪ لديهم تاريخ عائلي للمرض . أوضحت النتائج المتحصلة عليها أنه يوجد فرق ذو دلالة إحصائية في قيمة وقت التنشيط الجزئيوكانت قيمة وقت التنشيط الجزئي لتجلط الدم اعلى في المرضى الذين يعانون من خمول نشاط الغدة الدرقية )0.0(P مقارنة بالمجموعة الضابطة , بينما لاتوجد اختلافات ذو دلالة احصائية في قيم فحص وقت البرثرومبين ((P.value 0.849والنسبة المعيارية الدولية لوقت البرثرومبين (P.value (0576. مقارنة بالمجموعة الضابطة. وعلى حسب حدة مرض خمول نشاط الغدة الدرقية كانت نتائج )43( مريض يمثلون 86% متوسطة الي خفيفة وصنفت حسب نتائج(TSH≤40mU/L (TSH ونتائج (7) مرضى يمثلون 14% كان المرض حاد TSH> 40 mU /L).(وجدت الدراسة ان هنالك علاقة سالبة بين وقت النشيط الجزئي وهرمونات الغدةالدرقية ((T4, T3 .بينما لا توجد علاقة بين وقت البرثرومبين والنسبة المعيارية الدولية لوقت البروثرومبين مع هرمونات الغدة الدرقية .((T4, T3ووجدت الدراسة ان هنالك علاقة موجبة بين وقت النشيط الجزئي و)TSH(, بينما لاتوجد علاقة بين (TSH)ووقت البرثرومبين والنسبة المعيارية الدولية لوقت البروثرومبين .تخلص الدراسة ان وقت التنشيط الجزئي يزيد في حالة المرضى الذين يعانون من خمول نشاط الغدة الدرقية ولذلك توصي الدراسة بقياس وقت النشيط الجزئي لمرضى خمول نشاط الغدة الدرقية
