الملخص
Type 1 diabetes mellitus (T1DM) is a chronic disorder with well−known short− and long−term consequences. One of the long−term consequences is severe impairment of growth and development. Growth parameters are important indicators of a child's overall health, and they are influenced by factors like blood glucose control in diabetic children. This retrospective, cross-sectional, case control study was conducted at diabetes clinic for children and adolescents with Type One Diabetes at Wad Medani Paediatric Teaching Hospital, to study growth parameters of diabetic children in comparison with age-gender matched healthy controls and evaluate effect of dose of insulin, duration of disease, Level of Glycosylated Haemoglobin (HbA1c) and age at diagnosis of diabetes on growth. Eighty one diabetic children (boys: 45, girls: 36) and age gender matched healthy controls were enrolled. All subjects underwent anthropometric measurements (standing height and weight). Mean height (HAZ), weight (WAZ) and body mass index (BAZ) for age Z scores were calculated. Diabetes control was evaluated by measuring glycosylated haemoglobin (HbA1c). Statistical analysis was done by SPSS version 16. Diabetic children were shorter and lighter than controls. The mean height of diabetic boys was 143.03 (HAZ −1.82) and were significantly lower than that of controls 151.782 (HAZ –0.9356) (P < 0.001). The mean height for age Z scores (HAZ) of the diabetic girls was (-1.74) and were significantly lower than that of controls (-0.6750) (P < 0.040). The mean weight of boys was 35.87(WAZ-2.20) and were significantly lower than that of controls 40.9889(WAZ-0.60267) (P < 0.000). The mean weight of girls was 39.60 (WAZ-1.47) and were significantly lower than that of controls 44.4306 (WAZ- 0.4364) (P < 0.051). BMI for age Z scores (BAZ) were comparable in both groups. In our study we found that height and weight were positively correlated with age at onset of diabetes and dose of insulin , where negatively correlated with level of Hemoglobin A1c and duration of diabetes .Children diagnosed at younger age needed more attention to optimize growth.
