الملخص
In the present study, the urine and blood specimens were collected from 60 patients suffering from prostatitis and from (20) apparently healthy man as control in Al-Hussein Teaching Hospital in Thi-Qar province , Among the patients the prostatitis was occurred mainly at the age group ( 15-45 ) years in 34 (56.67 % ) of patients with significant differences ( p< 0.001 ) followed by the age group (>45-65) years in 17 (28.33 %) patients and the age group ( > 65 ) years in the 9 patients ( 15 % ) . The bacterial infections was detected in urine specimens by culture and PCR techniques after the prostatic massage. In the culture , the bacterial growth was detected in 11 (18.33%) of specimens including (10) isolates belong to Escherichia coli and (1) isolate of Enterobacter aerogenes . While , PCR technique by using universal 16S rRNA primer ( B27f-U1492r ) shown a positive bacterial signals in 23 (38.33%) of specimens with the high specificity and sensitivity than culture method ( p<0.001) . The incidence of Chlamydia trachomatis , Ureaplasma urealyticum and Mycoplasma genitalium was identified by using orf8 , ureA-B and mgpa gene primers respectively . M. genitalium was detected in 4 cases which represents (17.391 %) of cases followed by C. trachomatis in 3 cases which represents ( 13.043 %) and U. urealyticum was identified in 1 case (4.437 % ) of total specimens with high significant differences (p<0.001). While four (17.391%) PCR positive bacterial signals that detected by universal primer B27-U1492 were undiagnosed by all specific primers that used in the current study . According to the results of culture method and PCR techniques , the incidence of bacterial species in the clinical specimens of bacterial prostatitis revealed that E. coli was a common bacterial cause in 10 ( 43.47% ) cases with a high significant differences (P<0.001) , Followed by M. genitalium and C. trachomatis which diagnosed in 4 (17.39%) and 3 (13.04%) , respectively . The Enterobacter aerugenes and U. urealiticum were recorded in 1 ( 4.34% ) for each one with no significant differences among them . While four PCR positive cases by B27-U1492 16S rRNA only assigned as unknown bacteria . So, depending on the results above , bacterial prostatitis was diagnosed in 23 (38.33%) patients . While , nonbacterial prostatitis (CP/CPPS) was diagnosed in 37 (61.67%) of the patients . The isolated bacteria showed high sensitivity to amikacin where 9 ( 81.81% ) of isolates were sensitive to this antibiotic with high significant differences . While , the isolates shown moderate sensitivity to gentamicin and ciprofloxacin 4 and 3 ( 36.37 and 27.28 )% of isolates respectively and low antibiotic sensitivity was shown against cefotaxime and azithromycin (18.19%) for each one . All isolated bacteria exhibit completely resistance to ampicillin and co-trimoxazole , two isolates were shown resistance to all tested antibiotics . MIC were determined in the present study against susceptible isolates , CN has the lower MIC value ( 1.82 )μg/ml followed by AK (2.64) μg/ml . CTX has a highest MIC value (8.5) μg/ml compared with the other antibiotics . Virulence-associated genes, cnf-1 ,usp and kpsMTII in the isolated bacteria was detected in the current study , eight isolates appear to be enriched in kpsMTII ( 72.72 % ) of 11 isolates with the high significant differences (p < 0.001 ) , usp genes was identified in 2 isolates (18.18 % ) , while , cnf-1 was undetected in isolated bacteria . Serum cytokines levels for the prostatitis patients and control groups determined by ELISA , high serum concentration of IFN-γ was detected in bacterial prostatitis patients 15.88 ( ± 14.48) pg/ml followed by nonbacterial prostatitis group which records 11.69 (±8.44) pg/ml with high significant differences ( p<0.001 ) than in control group which recorded 5.43 (±1.11) pg/ml . IL-6 elevated in bacterial prostatitis patients 20.11 (±16.76) pg/ml compared with nonbacterial prostatitis patients which records 10.32 (±10.49)pg/ml and control group 3.18 (±1.37) pg/ml with high significant differences (p<0.001). The serum copper concentration was increased in both patients groups , in bacterial prostatitis patients recorded ( 134.51±44.18 ) μg/dl compared with the nonbacterial prostatitis patients which gave ( 127.96±40.24 ) μg/dl . While the control group was recorded ( 84.88±12.86 ) μg/dl with highly significant differences among these groups ( p< 0.001 ) . There is no significant differences shown in the serum zinc concentration among the groups in the current study that recorded ( 130.84±33.09 ) , ( 137.68±37.24 ) and ( 138.31±34.7 ) μg/dl for bacterial prostatitis , nonbacterial prostatitis and control group , respectively .
