الملخص

The Study of Cytokine Polymorphisms and Their Genotypes as Risk Factors Increasing Human Susceptibility to Viral Hepatitis Infection The objective of this work is to study cytokine polymorphisms and their genotyping associations as predictors of susceptibility to hepatitis infection. The main aims of this project were to investigate the associations between candidate gene variants and the risk for HBV and HCV infections, study the polymorphisms of some related genes with their allele frequencies and genotype associations with each type of infection, including some important cytokines such as interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), interleukin-10 (IL-10), Vitamin D receptors (VDR), transforming growth factor beta1 (TGF-β1), Na+-taurocholate co-transporting polypeptide (NTCP), apolipoprotein B mRNA editing enzyme catalytic polypeptide-like 3G (APOBEC3G), interleukin-28 (IL-28), and Angiotensin-converting enzyme Insertion/Deletion (ACE I/D), in a sample of patients affected by either HBV or HCV infections in addition to healthy individuals as controls. In addition, serum levels of some cytokines such as TGF-β, TNF-α, IL-2, IL-18, IL-1, and IL-6 were measured to evaluate differences between HBV and HCV patients as well as between patients and healthy controls. The distribution of APOBEC3G was higher in patients with hepatitis B (87.2%) and hepatitis C (82.5%) compared to healthy individuals (81.7%) (p-value = 0.1). In the ACE I/D gene, the ratio was highly significant: 17.07% in controls, 71.3% in HBV patients, and 80.73% in HCV patients (p-value = 0.0002). NTCP results showed no significant differences (p-value = 0.7). For IL-28B rs8099917, the TT genotype was higher in patients with hepatitis C and B compared to healthy individuals. The T allele was also higher than the G allele in all groups: 76.22% in controls, 65.85% in HBV, and 58.72% in HCV (p-value = 0.005). Polymorphisms of TNF-α 308 A/G showed that the GG genotype was higher: 73.17% in controls, 55.32% in HBV, and 58.72% in HCV. The G allele was higher than the A allele (84.76% in controls, 69.15% in HBV, and 76.61% in HCV) (p-value = 0.004). For IL-6 (174 promoter), the CG genotype was higher: 59.76% in controls, 57.45% in HBV, and 63.3% in HCV. The G allele was higher than the C allele (78.05% in controls, 70.2% in HBV, and 67.61% in HCV). IL-10 (1082) polymorphisms showed that the GA genotype was higher in HBV (46.81%) and HCV (42.2%), whereas GG was higher in controls (39.02%). Allele frequency analysis revealed that the G allele was higher in controls (57.32%), while the A allele was higher in HBV (62.77%) and HCV (58.72%) (p-value = 0.02). TGF-β1 (T29C) polymorphisms showed that the TC genotype was higher: 56.10% in controls, 54.26% in HBV, and 55.96% in HCV. The T allele was higher: 69.51% in controls, 57.98% in HBV, and 61.01% in HCV (p-value = 0.01). VDR polymorphisms showed that the CC genotype was higher: 58.54% in controls, 54.26% in HBV, and 56.88% in HCV. The T allele was higher: 79.27% in controls, 75.53% in HBV, and 72.02% in HCV (p-value = 0.0002). Hepatitis patients showed a different secretion profile of cytokines compared to healthy controls. Higher amounts of IL-1, IL-2, IL-6, IL-18, TGF-β, and TNF-α were secreted by patients. No correlation was observed between serum levels and patients’ age or gender. Overall, the mean of all measured criteria was higher in patients than in controls (p-value = 0.02).
