الملخص
Background: Anaemia and other haematological abnormalitiesoccur commonly in patients with smear positive pulmonarytuberculosis. This study is conducted to study the pattern of theseabnormalities, and to determine the correlation between thesehaematological abnormalities and the categories of tuberculosispatients.Methods:Seventy-six patients with smear positive pulmonarytuberculosis were selected randomly, and data were collected infour months, during the period from August to December 1999. Anautomatic multiparameter blood cell counter is used for estimationof Hb, RBC count, haematocrit, MCV, MCH, MCHC, neutrophil, lymphocyte, and platelets count. Eosinophil and monocyte wererecounted using a thin film stained with leishman stain. The bonemarrow was examined in 20 patients, selected randomly, by asenior consultant haematologist. Serum iron and transferrin weremeasured, and sturation of transferrin is calculated.Result:Anaemia occur in 82% of the patients, 7% of patients havesevere anaemia (Hb 155 mg/dl).34.5% presented with subnormal TIBC, the percentagesaturation of transferrin is above 45% in 38.4% of tuberculosispatients, which it is below 20% in only 15.4%.The bone marrow examination discloses moderatehypercellularity with active granulopoiesis. Erythropoiesis isnormoblastic in all patients, and no abnormal cells is detected. Conclusion:Anaemia is common in patients with pulmonary TB.Normochromic normocytic anaemia dominate the peripheral bloodpicture. The type and degree of anaemia is not related to theclinical categories of the tuberculosis patients.Anaemia in patients with pulmonary TB is attributedmainly to the reduction of circulating erythrocyte.Neutrophil leucocytosis and peripheral lymphopenia areremarkable features in patients with active pulmonary tuberculosis.Tuberculosis patients who default therapy present with markedleucocytosis and thrombocytosis comapred to the other clinicalcategories.Percantage saturation of total iron biding capacityapproaching values of iron deficiency anaemia had been observedto occur in 15.4% of patients. The bone marrow examination in all patients reflects activegranulopoiesis and normoblastic erythropoiesis. Iron is absent fromthe bone marrow in 10% of cases.
