Role of Genetic, Microbial and Immunological factors in male infertility

In this study, one hundred and fifty men (75 azoospermic (Azoospermia) men, 75 oligospermic (Oligospermia) men, and 25 fertile men as control (Normospermia)) were investigated for Azoospermia Factor (AZF) microdeletion, microbial infection, and antisperm antibody (ASA) titer. Seminal fluid, blood, and serum specimens were collected from each man to investigate infertility factors over a period of 1 year from February 2011 to February 2012. These seminal fluids of patients were submitted to the laboratory from the fertility clinics of Babylon Maternity and Children Hospital and private clinics. Each specimen was collected by the patient himself by masturbation. Two hundred and fifty-three isolates (single and mixed infections) were identified: Ureaplasma urealyticum 6.32%, Ureaplasma parvum 2.77%, Mycoplasma hominis 2.47%, Mycoplasma genitalium 7.51%, Chlamydia trachomatis 12.65%, Streptococcus pyogenes 11.07%, Staphylococcus aureus 14.23%, Staphylococcus epidermidis 15.42%, Staphylococcus saprophyticus 1.19%, Escherichia coli 25.69%. Infection with microorganisms was higher in azoospermic patients than in the normospermic group (control). There were significant differences in each type of infertile group (azoospermia and oligospermia) compared with the fertile group (normospermia); for the azoospermic group, the p-value < 0.05. The types of viral infections associated with azoospermia and oligospermia were detected and identified by the ELISA technique. For this study, seven common sexually transmitted viruses were chosen, as they can chronically infect the male genital tract, often without causing symptoms. Viral infection in azoospermia and oligospermia patients were as follows: rubella 12.67%, herpes simplex I 14.1%, herpes simplex II 34.1%, cytomegalovirus 20.42%, human papillomavirus 9.51%, adeno-associated virus 6.7%, Epstein-Barr virus 2.5%. In azoospermic and oligospermic patients, it was revealed that viral infection was higher when compared with normospermic men. The microdeletion percentage of AZFb and AZFc was higher than other AZF microdeletions, 34.91% and 36.96% respectively. Both AZFb and AZFc formed 71.87% of all AZF Y chromosome microdeletions. In this study, 75 azoospermic, 75 oligospermic, and 25 normospermic men had altered Y chromosome structure and were surveyed for AZFabc deletion. Y chromosome microdeletions for all groups were AZFa 179 (16.6%), AZFb 375 (34.9%), AZFc 397 (36.9%), AZFd 123 (11.4%). AZFa microdeletion among azoospermic, oligospermic, and normospermic men was 114 (10.61%), 57 (5.30%), and 8 (0.74%) respectively, while Y chromosome microdeletions in AZFb were: for azoospermic 86 (8%), oligospermic 266 (24.7%), and normospermic 23 (2.14%) respectively. On the other hand, microdeletions in AZFc were: for azoospermic 198 (18.4%), oligospermic 183 (17.0%), and normospermic 16 (1.48%). Y chromosome microdeletions in AZFd were: for azoospermic 63 (5.86%), oligospermic 56 (5.21%), and normospermic 4 (0.37%). Antisperm antibody (ASA) related to patient groups was as follows: azoospermia 22 (66.7%), oligospermia 9 (27.2%), and normospermia 2 (6.1%). The results of this study of infertile patients (azoospermia and oligospermia) formed the largest ratio of raised ASA titer compared to the control group (93.9% and 6.1%, respectively), which can explain the defect in the number and activity of sperm in azoospermic and oligospermic patients. Also, the highest value of ASA among all groups was shown in azoospermic patients within the AZFc subgroup, 14 (63.64%) cases, followed by AZFa 7 (31.82%) cases.
