INFECTIONS URINAIRES AU CHU FRANTZ FANON DE BLIDA: ASPECTS EPIDEMIOLOGIQUES ET BACTERIOLOGIQUES.

Les infections urinaires sont des pathologies fréquentes aussi bien en communauté qu’à l’hôpital, dont l’expression clinique est de gravité et de symptomatologie variées.Nous avons mené une étude rétrospective durant une période de sept ans s’étendant du 012012 au 31 Décembre 2018, portant sur tous les ECBU positifs réalisés au laboratoire central de biologie du CHU Frantz Fanon, Blida. Le but de ce travail était de déterminer la fréquence des infections urinaires, d’étudier l’épidémiologie et la bactériologie des germes responsables de ces infections urinaires, ainsi que le suivi de l’antibiorésistance. avons constaté que sur 17 515 prélèvements d’urine reçus, 2941 répondaient aux critères d’infection urinaire, soit 16,79 % de positivité. Toutes les tranches d’âges ont été touchées par cette infection avec une prédominance du sexe féminin (69 %).Les germes responsables sont surtout les bactéries à Gram négatif 84,12 %, largement dominés parEscherichia coli (52,33%), suivi de Klebsiella pneumonaie (13,16 %), Proteus mirabilis (03,60 %), etPseudomonas aeruginosa (03,43 %). Les bactéries à Gram positif représentaient respectivement 04,11% et 02,45 % pour les Enterococcus faecalis et Streptocoques bêta hémolytique groupe B. Nous avons noté une résistance accrue d’Escherichia coli vis-à-vis des différentes famillesd’antibiotiques largement prescrits notamment l’ampicilline (73 %) et l’association amoxicilline-acideclavulanique (32 %), ainsi que le cotrimoxazole (46 %), l’ertapénème et l’imipénème gardentrelativement encore une bonne activité. prévalence des infections urinaires à entérobactéries productrices des BLSE dans notre étude était 11,78 %, parmi toutes les infections urinaires à entérobactéries. La résistance des bactéries isolées aux antibiotiques usuels, mise en évidence dans notre étude, montrel’intérêt du suivi de l’évolution de l’écologie bactérienne et du profil de résistance. Ces résultatssuggèrent la nécessité de la gestion de la prescription des antibiotiques, tout en adaptantl’antibiothérapie à l’antibiogramme, au patient et à son environnement.

- Urinary tract infections are common pathologies both in the community and in hospitals, and their clinical expression is of varying severity and symptomatology. We conducted a retrospective study over a seven-year period from January 1, 2012 to December 31, 2018, covering all, cytobacteriological examination of urine, positive results from the central biology laboratory of the Frantz Fanon University Hospital, Blida. The purpose of this work was to determine the frequency of urinary tract infections, to study the epidemiology and bacteriology of the germs responsible for these urinary tract infections, as well as the monitoring of antibiotic resistance.Of the 17,515 urine samples received, 2941 met the criteria for urinary tract infection, or 16.79% positive. All age groups were affected by this infection with a predominance of the female sex (69%). The germs responsible are mainly Gram-negative bacteria84.12%, largely dominated by Escherichia coli (52.33%), and followed by Klebsiella pneumonia (13.16%), Proteus mirabilis (03.60%), and Pseudomonas aeruginosa (03.43%). Gram-positive bacteria accounted for 04.11% and 02.45% respectively for Enterococcus faecalis and Streptococcus beta hemolytic group B. We have noted increased resistance of Escherichia coli to the different families of widely prescribed antibiotics including ampicillin (73%) and the combination of amoxicillin and clavulanic acid (32%), as well as cotrimoxazole (46%), ertapenem and imipenem still maintain relatively good activity.
