الملخص
Inflammatory bowel disease (IBD) is a group of gastrointestinal disorders that cause prolonged and chronic inflammation of the gastrointestinal tract. The major IBD phenotypes are Crohn’s disease (CD) and ulcerative colitis (UC). Ulcerative colitis is a disease of unknown etiology which is characterized by inflammation of the mucosa and sometimes the submucosa of the colon, while Crohn’s disease causes transmural inflammation that affects any portion of the GIT (commonly the terminal ileum or the perianal region) in a non-continuous type. The obtained results compared to the healthy group indicate. This study was conducted from November 2020 till March 2021. The entire samples were collected from people who attended Marjan Medical City in Babylon Province, Hilla City. The laboratory work was done in the Department of Chemistry and Biochemistry, College of Medicine, University of Babylon. The presented study aims to evaluate the levels of Zinc, Selenium, Vitamin D, and Vitamin B12 and fecal calprotectin in patients with Inflammatory Bowel Diseases by measuring serum levels and comparing the results with those of the control group in Babylon-Iraq, to find the correlation of Zinc, Selenium, Vitamin D, and Vitamin B12 and fecal calprotectin with lipid profile and other parameters in these patients and compare them with apparently healthy individuals as a case–control study. The study included (50) patients as a patient group diagnosed with IBD (G1) and (50) as an apparently healthy control group (G2). The age of all studied groups was 14 years old or above, and BMI was (18.5–24.9) Kg/m². Serum was used in the determination of fasting blood sugar (FBS), lipid profile, C-reactive protein (CRP), Vitamin D, Vitamin B12, Zinc, and Selenium, and whole blood was used to determine ESR level, while stool samples were taken for the fecal calprotectin test. The results revealed a decrease in Total Cholesterol (TC), Low-Density Lipoprotein Cholesterol (LDL-c), and High-Density Lipoprotein Cholesterol (HDL-c) in patient groups compared to healthy controls. FBG and Triglyceride (TG) were elevated in the patient group compared to the control group. The results showed a significant decrease in Vitamin D, Vitamin B12, Zinc, and Selenium levels in the patient group compared to the control group, while ESR, CRP, and fecal calprotectin showed significant elevation in the patient group compared to the control group. A significant positive correlation was found between Vitamin D, Vitamin B12, Zinc, Selenium, and HDL, and a significant negative correlation was found between Vitamin D, ESR, TG, and fecal calprotectin, while a non-significant correlation was found in the patient group between Vitamin D, TC, and LDL-c. A significant positive correlation was found between Zinc and Vitamin B12, Vitamin D, Selenium, and HDL, and a significant negative correlation was found between Zinc, ESR, TG, FBS, and fecal calprotectin, while a non-significant correlation was found in the patient group between Zinc, TC, and LDL-c. A significant positive correlation was found between Vitamin B12, Selenium, and HDL, and a significant negative correlation was found between Vitamin B12, ESR, TG, and fecal calprotectin. A significant positive correlation was found between Selenium and HDL, and a significant negative correlation was found between Selenium, ESR, TG, FBS, and fecal calprotectin. A significant positive correlation was found between fecal calprotectin, FBS, TG, and ESR, and a significant negative correlation was found between fecal calprotectin and HDL. The conclusion from this study found that: The Vitamin D concentration in patients with IBD should be examined routinely, since IBD constitutes a risk factor for Vitamin D deficiency. Zinc levels are often low in patients with chronic diarrhea or malabsorptive disorders. Selenium deficiency is a common finding in patients with IBD and correlates with disease severity. Inflammatory bowel disease patients may be at risk of Vitamin B12 insufficiencies. Fecal calprotectin is an ideal non-invasive biomarker to identify intestinal inflammation throughout the gastrointestinal tract.
