Thromboembolic Phenomenon In Patient With Mitral Stenosis In Ahmed Gasim Hospital

الملخص

Rheumatic mitral stenosis is a common problem in our community,commonly affecting the young population. It can be a crippling diseasewith many complications among which thromboembolsim is one of themost serious.The aim of this study was to study the prevalence, and identify therisk factors for thromboembolic problems in Sudanese patient with mitralstenosis seen at Ahmed Gasim cardiac center.Records of patients admitted from Jan 1999-Jan 2003, were reviewedregarding many clinical and echocardiographic variables. A total of 412patients were studied, 142 (34.5%) were males, 270 (65.5%) were females.There were no significant differences in the gender P = (0.606) orseverity of symptoms between patients with and withoutthromboembolism. Patients with thromboembolism were older(P<0.016), had more frequent atrial fibrillation P = (0.017), smaller mitralvalve area P= (0.021), increased pulmonary artery pressure P = (0.012) ascompared to patients without clot.Dilated left atrial size was significantly associated withthromboembolism P= (0.034), spontaneous echo contrast P= (0.048) andthe presence of intracardiac thrombus P= (0.029). The presence ofintracardiac thrombus was significantly associated with boththromboembolism P= (0.05) and spontaneous echo contrast P= (0.000).10Transoesophageal echo is more sensitive in detecting both spontaneousecho contrast P= (0.000) and intracardiac thrombus P = (0.000) thantransthoracic echo.These results high light the clinical and echo risk factors forthromboembolism, the sensitivity of transoesophageal echo forassessment of these patients, and the need for early referral for PTMC.
