La prise en charge des hydrocéphalies sur tumeurs de la FCP est devenue une préoccupation actuelle de tout neurochirurgien de choisir entre la dérivation ventriculo-péritonéale (DVP), et la ventriculocisternostomie (VCS), celle-ci permet de gérer l’hydrocéphalie vue le taux de succès rapporté. Nous avons effectué une étude sur 42 patients présentant tous une hydrocéphalie secondaire aux tumeurs de la FCP, dont 21 cas traités par DVP, et 21 cas par une VCS, entre 2002 et 2010, au service de neurochirurgie du CHU Mohamed VI de Marrakech. Dans notre contexte les tumeurs de la FCP s’accompagnant de l’hydrocéphalie représente 7,13% par rapport à l’ensemble des tumeurs de cette région. L’âge moyen des patients traités par DVP était de 13 ans versus 27 ans dans le groupe des VCS, dont la majeure partie était des adultes dans 61,91% et les enfants dans 38,09% avec une légère prédominance féminine dans 57,14% versus 52,38% des cas traités par DVP étaient des enfants, les adultes dans 47,62%, avec une prédominance masculine dans 61,9%.La symptomatologie clinique ayant révélé la pathologie au sein des 2 groupes était dominée par le syndrome d’HTIC dans 85,71%, un syndrome cérébelleux dans 64,90%, troubles de conscience, une macrocranie dans 9,52% et une atteinte des paires crâniennes essentiellement la 2ème, 3èmeet la 6ème paire crânienne. Le bilan radiologique est basé sur l’association TDM, IRM dans 85,71%. Les étiologies tumorales les plus trouvées étaient les épendymome, les astrocytome et les médulloblastomes. L’évolution de nos patients était marquée par la survenue de complications dans 19,04% du groupe VCS versus 38,1% du groupe DVP. Les complications hémorragiques les plus fréquentes dans 9,52 % dans le groupe VCS versus les complications mécaniques dans 23,80%, le taux de succès de la VCS est de 76%, un faible taux de mortalité à 4,76%, versus 9,52% dans le groupe DVP.

- The management of hydrocephalus in tumors of the posterior fossa (PF) has become a current concern of any neurosurgeon to choose between ventriculo-peritoneal shunt (VPS), and the third ventriculostomy (ETV), it can manage the hydrocephalus to rate success reported. We conducted a study of 42 patients with hydrocephalus secondary to tumors of the PF, including 21 cases treated with VPS, and 21 cases by a ETV, from 2002 to 2010, at the neurosurgery department of university hospital marrakesh Mohamed IV. In our context tumors PF accompanied by hydrocephalus represents 7.13% on all tumors of this region. The average age of patients treated with VPS was 13 years versus 27 years in the group of ETV, most of which was 61.91% in adults and children in 38.09% with a slight female predominance in 57,14% versus 52, 38% of cases treated with VPS were children, adults in 47.62%, with a male predominance in 61.9%. The clinical test showing the pathology in the two groups was dominated by the Sd of intracranial hypertension in 85.71%, a 64.90% in cerebellar Sd, impaired consciousness, a macrocrania in 9.52% and the achievement of cranial pairs essentially the second, third and sixth cranial nerve. Radiological assessment is based on the combination CT, MRI in 85.71%. The tumor causes the most ependymoma were found, the astrocytoma and medulloblastomas. The evolution of our patients was marked by the occurrence of complications in 19.04% of the ETV group versus 38.1% of the VPS group. The most common bleeding complications in 9.52% in the ETV versus mechanical complications in 23.80%, the success rate of the ETV is 76%, a low mortality rate to 4.76%, versus 9,52% in the DVS.
